Current Concepts in Rectal Cancer

Abstract: Since the first recorded surgical resection for rectal cancer in 1826 by Jacques Lisfranc, 1 the management of rectal cancer has evolved remarkably with dramatic improvements in local recurrence and mortality. Surgical resection is the only proven curative treatment of rectal cancer and the important advances in care have involved improvements in surgical techniques. Most of these improvements during this time period resulted from a better understanding of the anatomy and pathology of rectal cancer. It is impo… Show more

“…In China, CRC is the fourth most common cancer and the fifth leading cause of death among all types of human cancer [2, 3]. Although CRC has a very high cure rate following surgical resection followed by multidisciplinary treatment (radiation and systemic chemotherapy) in the early stages, the high rate of relapse and distant metastasis still threaten a large percentage of patients [4, 5]. Efforts have been made to elucidate the genetic and molecular characteristics of CRC, for prognosis and response to targeted treatment [3, 5].…”

“…Surgical removal of colon cancer is the most effective therapeutic approach but it requires early diagnosis [ 19 ]. In advanced stages of the disease, chemotherapy and radiotherapy are other alternative approaches [ 20 , 21 ].…”

“…Along with surgical advances such as the total mesorectal excision (TME), the introduction of chemoradiotherapy (CRT) has significantly lowered the local recurrence rate from 15%–20% to 4%–5% [1]. However, the TME with CRT is associated with a mortality rate of 1%–4% and a morbidity, including bowel and urogenital dysfunction, rate of ~40%.…”

“…The surgical approach for treating patients with rectal cancer has evolved from the perineal resection to sphincter-saving surgery so as to decrease local recurrence and to preserve organs. Along with surgical advances such as the total mesorectal excision (TME), the introduction of chemoradiotherapy (CRT) has significantly lowered the local recurrence rate from 15%–20% to 4%–5% [ 1 ]. However, the TME with CRT is associated with a mortality rate of 1%–4% and a morbidity, including bowel and urogenital dysfunction, rate of ~40%.…”
