IntrODuçãOAs verrugas anogenitais são causadas pelo papilomavírus humano (HPV), e são transmitidas principalmente pela via sexual. Relatos médicos indicam um aumento do acometimento das lesões HPV-induzidas em crianças, sendo os tipos 6 e 11 os mais comumente encontrados. Em meninas as lesões apresentam distribuição vulvar, vaginal, uretral e perianal. Elas apresentam três vezes mais chance de cursar com verruga anogenital do que os meninos 1,2 . A via de transmissão infantil ainda não está clara. Especula-se sobre a transmissão vertical, da mãe para o filho, através da infecção transplacentária e do líquido aminiótico; a transmissão perinatal, durante o parto através do trato genital contaminado; e ainda a transmissão horizontal, através da manipulação da criança por mãos infectadas (heteroinoculação), a autoinoculação ou, indiretamente, por fômites contaminados. Como existe a possibilidade de a transmissão ter ocorrido por abuso sexual, essa forma de contaminação deverá ser pesquisada 2,3 . Até o momento, nenhum tratamento existente mostrou-se totalmente eficaz no que se refere à eliminação viral. Os tratamentos tradicionais realizados através de excisão cirúrgica ou da utilização de métodos físicos e químicos são dolorosos, requerem internação e anestesia, podendo deixar cicatrizes com extensão proporcional ao tamanho da lesão. Uma alternativa ao tratamento clínico seria o uso do imiquimode creme a 5%, que possui atividade imunomoduladora, com estimulação da imunidade celular, responsável pela regressão da lesão. Este medicamento já é utilizado com eficácia no tratamento de lesões HPV-induzidas em adultos.Sabe-se que a longa duração para a resolução das verrugas genitais indica um fraco reconhecimento das células infectadas pelo sistema imunológico, sendo a imunidade celular o mecanismo essencial para a regressão da lesão.O imiquimode é uma imidazoquinolina de baixo peso molecular. Menos de 0,9% do fármaco aplicado de forma tópica é absorvido. É uma droga excretada por via urinária e fecal na proporção de 3 para 1, que possui meia-vida elevada por retenção prolongada na pele. A ação imunomodulatória do imiquimode consiste na estimulação da imunidade celular, pela forte resposta Th1 e aumento do número de células T citotóxicas, induzindo os queratinócitos a produzirem citocinas como o interferon-alfa (IFN-α) e a interleucina-12 (IL-12) e ativando as células de Langerhans, melhorando a apresentação de antígenos para as células T 4 . OBJEtIVO PrimárioPromover tratamento não mutilador para crianças portadoras de verrugas anogenitais com o uso do imiquimode creme a 5%. SecundárioAvaliar os efeitos locais do uso do imiquimode, enfocando as reações adversas e a tolerabilidade; e a durabilidade da resposta clínica decorrente da eliminação viral. Atopic dermatitis is a major allergic disease that develops through dysregulation of Th2-mediated inflammation. Although dendritic cells (DCs) have been thought to play a critical role in the upstream phase of the allergic cascade, conventional drugs such as steroids and chemical mediator antagonists target the effector cells or factors in allergic inflammation. Recently, it has been demonstrated that interaction between thymic stromal lymphopoietin (TSLP) and human DCs plays an essential role in evoking inflammatory Th2 responses in allergy through OX40 ligand expression on DCs. In this study, we provide evidence that R848, an imidazoquinoline compound, which is a TLR ligand and a strong Th1 response-inducing reagent, is a potent adjuvant for the alteration of the Th2-inducing potency of human DCs activated by TSLP (TSLP-DCs). R848 inhibited the inflammatory Th2-inducing capacity of TSLP-DCs and redirected them to possessing an IL-10 and IFN-γ-producing regulatory Th1-inducing capacity. This functional alteration depended on both repression of OX40 ligand expression and induction of IL-12 production from DCs by the addition of R848. Additionally, R848 had the ability to inhibit the TSLP-mediated expansion and maintenance of the Th2 memory response. These findings suggest that imidazoquinoline may be a useful in the treatment of allergic diseases that are triggered by TSLP.

Imiquimod (IQ) is an immune-response modifying agent, fi rst approved by FDA for the topical treatment of external genital and perianal warts in 1997. It induces, through stimulation of Toll-like receptors (TLRs) localized on the surface of antigen-presenting cells, synthesis and release of several endogenous pro-infl ammatory cytokines such as interferon-α (IFN-α), tumor necrosis factor-α (TNF-α) and interleukins (IL) 6 and 12, which in turn stimulate both the innate and acquired immune pathways, resulting in upregulation of natural antiviral and antitumor activity. IQ 5% cream has been used for the treatment of a wide variety of dermatologic conditions in which the immune system is thought to play a role in regression of the disease. In some disorders, such as genital and perianal warts, actinic keratoses, basal cell carcinomas, Bowen's disease and molluscum contagiosum, relative safety and effi cacy are supported by randomized controlled trials of IQ. However, it is common for patients to experience local skin reactions, which can range from mild to severe in intensity, but usually resolve 1-2 weeks after interrupting treatment. Additional randomized trials are encouraged to assess safety and effi cacy of IQ in the treatment of an even wider range of cutaneous disorders.
