-Background -Helicobacter pylori (H. pylori) gastric infection is a main cause of inflammatory changes and gastric cancers. Objective -The aim of this study was finding the effects of curcumin on oxidative stress and histological changes in chronic gastritis associated with H. pylori. Me thods -In a randomized clinical trial, patients were divided into two groups: a standard triple therapy group and triple therapy with curcumin group. Endoscopic and histological examinations were measured for all patients before and after 8 weeks. Results -Triple therapy with curcumin treatment group significantly decreased malondialdehyde markers, glutathione peroxides and increased total antioxidant capacity of the gastric mucosa at the end of study compared to baseline and triple regimen groups. In addition, the oxidative damage to DNA was significantly decreased in triple therapy with curcumin group at the end of study compared to baseline and compared to triple therapy (P<0.05 for both). Triple therapy group in combination with Curcumin significantly decreased all active, chronic and endoscopic inflammation scores of patients compared to the baseline and triple therapy group (P<0.05 for both). The eradication rate by triple therapy + curcumin was significantly increased compared to triple therapy alone (P<0.05). Conclusion -Curcumin can be a useful supplement to improve chronic inflammation and prevention of carcinogenic changes in patients with chronic gastritis associated by H. pylori. HEADINGS -Gastritis. Objective: This study was designed to explore the benefit of curcumin as adjuvant therapy to the standard Helicobacter pylori eradication triple therapy in both duodenal and gastric ulcers patients.Methods: The present study enrolled 40 patients newly diagnosed endoscopically with peptic ulcer disease to be allocated into group 1 treated with standard H. pylori eradication triple therapy, and group 2 patients treated with curcumin (500 mg) capsules three times daily for 14 days as an adjuvant to standard triple therapy. Stool antigen test, immunoglobulin M serology test, tumor necrosis factor-alpha (TNF-α), interleukin 1 beta (IL-1β), and total antioxidant capacity (T-AOC) are measured at the baseline and after 6 weeks of treatment. Results:The result showed that the use of curcumin as adjuvant therapy produced highly significant improvement in healing efficacy which was significantly distinguished in duodenal ulcer patients compared to gastric ulcer groups 2 patients (p<0.05), along with a highly significant reduction in pro-inflammatory IL-1β level in group 2 patients (p<0.01). After 6 weeks of treatment, there was a highly significant elevation in the level of TNF-α in groups 1 (p<0.01), though group 2 patients presented with non-significant elevation in TNF-α level. Moreover, the T-AOC was improved with curcumin adjuvant therapy, though non-significant, compared to group 1 patients who showed a reduction in T-AOC. Conclusion:This study revealed that addition of curcumin as adjuvant therapy produced improvement in ulcer healing efficacy, and controlled the inflammatory and oxidative stress process induced by H. pylori infection.

Objective: The ethanolic extract of Vitis vinifera leaves (VVE) (500 mg/kg body weight), ranitidine (50 mg/kg body weight) and both of them were tested for their gastroprotective and curative effects against the incidence of peptic ulcer. Methods:The antiulcer effects of VVE were investigated using a combination of indomethacin and cold-stress for 2h. To ascertain the mechanism of action of VVE, its protective and curative effects were studied on gastric volume, gastric juice acidity, ulcer index and malondialdehyde (MDA), glutathione (GSH), catalase (CAT), glutathione-S-transferase (GST), superoxide dismutase (SOD), nitric oxide (NO) activities of both stomach and duodenum of rats. Moreover, histopathological effects on stomach and duodenum were determined. Results:The antioxidant activity of VVE was demonstrated using 2, 2-diphenyl-1-picrylhydrazyl (DPPH) test. VVE was found to reduce the ulcer index, gastric volume and the level of gastric juice acidity. Also, VVE showed gastroprotective and curative activities mainly through improvement of antioxidant status and decreasing lipid peroxidation accompanied with amelioration of both stomach and duodenum architectures. Conclusion:The prophylactic and curative effects of VVE proved to be effective in preventing gastric and duodenal ulceration which may be probably due to its antioxidant and anti-acid secretory effects.
