Guided Act and Feel Indonesia – Internet-based Behavioral Activation Intervention for Depression in Indonesia: A Systematic Cultural Adaptation

Depression is one of the leading causes of the global disease burden, affecting millions of people worldwide. The availability of mental health treatment, however, remains very limited in many low-middle income countries, including Indonesia. Internet-based interventions are known to have the potential to deliver mental health treatment economically and appropriately according to numerous studies conducted in high-income countries. In the current study, we describe a systematic cultural adaptation of an internet-based behavioral activation intervention for depression in Indonesia, named Guided Act and Feel Indonesia. During the adaptation, relevant stakeholders were involved, including licensed clinical psychologists, mental health communities, lay counselors, and patients. The adaptation used the formative method for adapting psychotherapy (FMAP) to adapt eight important cultural elements (language, persons, metaphors, content, concepts, goals, methods, and context). The intervention was adapted from the original Dutch version called Doe en Voel, consisting of 8 structured modules that are offered in a secure online environment. The adapted version is being delivered during an ongoing randomized controlled trial with non-face-to-face support from lay counselors who work under the supervision of licensed clinical psychologists. Namun, ketersediaan perawatan kesehatan mental masih sangat terbatas di negara-negara yang berpenghasilan rendah termasuk Indonesia. Kemudian, intervensi depresi berbasis internet telah diketahui memiliki potensi untuk memberikan perawatan kesehatan mental secara ekonomis dan tepat sesuai dengan berbagai penelitian yang dilakukan di negaranegara berpenghasilan tinggi. Pada studi ini, peneliti mendeskripsikan adaptasi budaya secara sistematis dari intervensi aktivasi perilaku berbasis internet untuk depresi di Indonesia, bernama Guided Act and Feel Indonesia. Selama proses adaptasi, para pemangku kepentingan yang relevan dilibatkan, termasuk psikolog klinis berlisensi, komunitas kesehatan mental, konselor, dan pasien. Adaptasi menggunakan metode formatif untuk adaptasi psikoterapi (FMAP) untuk mengadaptasi delapan elemen adaptasi budaya penting (bahasa, orang, metafora, konten, konsep, tujuan, metode, dan konteks). Intervensi ini diadaptasi dari versi asli Belanda yakni Doe en Voel, yang terdiri dari 8 modul terstruktur yang ditawarkan pada lingkungan online yang aman. Versi yang disesuaikan kemudian disampaikan selama percobaan terkontrol secara acak yang berlangsung tanpa tatap muka dari konselor yang bekerja di bawah pengawasan psikolog klinis berlisensi. Tantangan yang dihadapi selama adaptasi budaya juga dibahas dalam penelitian ini.

The Diabetes Prevention Program (DPP) has been shown to prevent type 2 diabetes through lifestyle modification. The purpose of this study was to describe the literature on DPP translation, synthesizing studies using cultural adaptation and implementation research. A systematic search was conducted. Original studies evaluating DPP implementation and/or cultural adaptation were included. Data about cultural adaptation, implementation outcomes, and translation strategies was abstracted. A total of 44 were included, of which 15 reported cultural adaptations and 38 explored implementation. Many studies shortened the program length and reported a group format. The most commonly reported cultural adaptation (13 of 15) was with content. At the individual level, the most frequently assessed implementation outcome (n=30) was adoption. Feasibility was most common (n=32) at the organization level. The DPP is being tested in a variety of settings and populations, using numerous translational strategies and cultural adaptations. Implementation research that identifies, evaluates, and reports efforts to translate the DPP into practice is crucial. Diabetes and its complications are largely preventable [2,3]; obesity, physical inactivity, and unhealthy eating account for over half of new cases [4,5].Racial/ethnic minorities are at substantially higher risk for type 2 diabetes and continue to experience greater rates of hospitalization due to diabetes-related complications and 50-100 % higher morbidity and mortality than their white counterparts [6][7][8]. Background: eHealth interventions have the potential to address challenges related to access, service engagement, and continuity of care in the delivery of mental health services. However, the initial development and evaluation of such interventions can require substantive amounts of financial and human resource investments to bring them to scale. Therefore, it may be warranted to increase policy, services, and research attention on eHealth platforms that have the potential to be adapted for use across settings. Yet, limited attention has been placed on the methods and processes for adapting eHealth interventions to improve their applicability across cultural, geographical, and contextual boundaries.Objective: In this paper, we describe an adaptation framework and protocol to adapt an eHealth intervention designed to promote recovery and prevent relapses in youth receiving specialized services for first-episode psychosis. The online platform, called Horyzons, was originally developed and tested in Australia and is being prepared for a wider evaluation in Canada.Methods: Service users and service providers from two specialized early intervention programs for first-episode psychosis located in different provinces will explore a betaversion of the eHealth intervention through focus group discussions and extended personal explorations to identify the need for, and content of contextual and culturaladaptations. An iterative consultation process will then take place with service providers and users to develop and assess platform adaptations in preparation for a pilot study with a live version of the platform. Running Head : Cultural and Contextual Adaptation of an eHealth InterventionResults : Data collection has been completed August 2017 and analysis is in process.Conclusions: This protocol contributes to an important gap in the literature pertaining to the specific principles, methods and steps involved in conducting a systematic reflection and change process in scaling up the evaluation of eHealth interventions across a diverse range of healthcare settings.
