ARTICLE Hackathon as a way to raise awareness and foster innovation for stroke Hackathon como uma maneira de aumentar a conscientização e promover a inovação no AVCThe World Stroke Campaign is a global effort to bring awareness about the burden of stroke, with activities scheduled annually on October 29th 1 . The campaign often has relied on traditional educational approaches, including lectures and media engagement. More creative approaches to attract attention have also been tried, such as celebrity endorsement, running/walking events, soccer events, musical concerts, and car races. Every year, the movement generates more attention worldwide, and the Brazilian Stroke Network 2 demonstrates this progress clearly.Campaigns are mostly carried out by professionals from the academy, which have attracted students to the cause. Student involvement creates an opportunity to educate future professionals, who will influence their future work and civic environments and generate ongoing momentum. Yet while the number of student associations participating has increased over the years, most are focused on health science. Ideally, other students would also be involved, because increasingly the model for addressing health issues is based on a multidisciplinary approach.We are overwhelmed with advanced technology, gadgets and mobile apps today, with countless possibilities for their applications in the health industry, from nanotechnology to semiconductors to Cloud technologies. In this fast-paced technological environment, high-tech companies and more recently science organizations have sought to develop AbstrACtThe 'Time is Brain" concept has been a major focus in the World Stroke Awareness Campaign. Objective: We describe our experience with a Hackathon (junction of hack+marathon), a 24 hour event of innovation-and technology-based project development, focused around detecting stroke. Method: The Hackathon was held during a Brazilian Student's union (BRASA) congress involving students from engineering and other technological disciplines. Results: Four teamscompleted the 24-hour challenge; two teams opted the stroke challenge. One team proposed improving detection using a mobile sensor Fit-Bit combined with a smart-phone to contact emergency dispatch. The other team developed a game, in which kids would play a role as a neurologist; to diagnosis stroke with objective to increase awareness. a b s t r a c tHypertension is known to affect around one third of adults globally and early diagnosis is essential to reduce the effects of this affliction. Today's Blood Pressure (BP) monitoring cuffs are obtrusive and inconvenient for performing regular measurements, and continuous non-invasive blood pressure devices are too complex and expensive for ambulatory use. Hence, there is a strong need for affordable systems that can measure blood pressure (BP) variations throughout the day as this will allow to monitor, diagnose and follow-up not only patients at risk, but also healthy population in general for early diagnosis. A promising method for arterial BP estimation is to measure the Pulse Transit Time (PTT) and derive pressure values from it. However, current methods for measuring this surrogate marker of BP require complex sensing and analysis circuitry and the related medical devices are expensive and inconvenient for the user. In this paper, we present new methods to estimate PTT reliably and subsequently BP, from the baseline sensors of smartphones. This new approach involves determining PTT by simultaneously measuring the time the blood leaves the heart, by recording the heart sound using the standard microphone of the phone, and the time it reaches the finger, by measuring the pulse wave using the phone's camera. We present algorithms that can be executed directly on current smartphones to obtain clean and robust heart sound signals and to extract the pulse wave characteristics. We also present methods to ensure a synchronous capture of the waveforms, which is essential to obtain reliable PTT values with inexpensive sensors. Additionally, we combine Autocorrelation and Fast Fourier Transform (FFT)-based methods for reliably estimating the user heart rate (HR) from his/her heart sounds, and describe how to use the calculate HR to compensate for the camera frame rate variations and to improve the robustness of PTT estimation. Our experiments show that the computational overhead of the proposed processing methods is minimum, which allows real-time feedback to the user, and that the PTT values are fully accurate (beat-to-beat), thereby enabling state-of-the-art smartphones to be used as affordable medical devices.

Thrombolysis with intravenous rt-PA is an effective recanalizing treatment for ischemic stroke within 4 and half hours from its onset 1 . It is clear that by providing the treatment earlier the outcome is better with fewer complications 2,3 . A pooled study 4 demonstrated that short stroke-onset-to-treatment time (OTT) with rt-PA had better functional outcomes (modified Rankin Scale, mRS, score of 0 to 1) than patients treated beyond the 90 minute OTT, with odds of 2.8 (95%CI 1.8-4.5) for OTT between 0 to 90 min in comparison to odds of 1.6 (1.1-2.2) for OTT between 91 to 180 min. A more recent pooled reanalysis with additional data showed similar findings 5 ; the number of patients needed to treat (NNT) to achieve an mRS score of 0 to 1 for OTT within 90 minutes is 4.5, whereas the NNT doubled to 9 for OTT between 91-180, and to 14 for OTT between 181-270 minutes.Therefore, the goal for an optimal treatment should be to target an OTT under 90 minutes, as pictured in Figure 1. Based on the recommended American Heart Association (AHA) stroke guideline (the text and references 6,7 in Figure 1 are derived from it 8 ), the time limits for pre-hospital (30 minutes) plus hospital care (60 minutes) fill up the 90 minute window. Experience has demonstrated that process improvement can lower the time spent in the pre-hospital 9 and hospital 10,11,12,13 phases, and educational awareness campaigns can improve stroke recognition and time to seek help 8 . Therefore, combining best practices and available technologies, it is possible to respond within 90 minutes. The strategy and plan of action require commitment from the stakeholders, including society and in particular patients at risk. We need to move forward to interpret the 60 minute door to needle recommendation as more than a time to attain. Yet few patients currently are treated in this time frame. From the standpoint of process improvement or a lean thinking perspective, there is a potential opportunity to reduce the time by eliminating non-value-added steps in each element of the stroke survival chain. The reduction in one time element does not necessarily shift the OTT under 90 minutes. Most likely, the reduction in OTT requires a coordinated approach to track and improve all elements of OTT, from the patient's ability to recognize the onset of stroke up to delivery of medication.
