CT Signs of Hepatofugal Portal Venous Flow in Patients with Cirrhosis

Abstract: A diameter of less than 1 cm for the main portal vein is highly specific, although not sensitive, for hepatofugal portal venous flow in patients with cirrhosis. This sign may be useful when sonography is limited, or this sign may prompt sonographic assessment in patients not known to have hepatofugal flow.

“…Since we only consider these shunts as present or absent, quantification of these shunts as well as other portosystemic collateral sites was not made, as a consequence significant association might not be detected. Bryce et al showed in their study that if spontaneous splenorenal shunts is present, main portal vein diameter is reduced which may be an indicator of hepatofugal flow in cirrhotic liver [5]. In our study, we paid attention to splenorenal shunts because of causing decreased outflow to the HV with regard to diminished portal perfusion.…”

“…Our study showed that there is a significant association between the hepatofugal flow in the portal vein and the impaired visualization of the HV. Splenorenal shunts are generally thought to drop the perfusion of the liver substantially [5]. However, the association between splenorenal shunts as independent variable and the impaired visibility of the HV could not reach the significant level.…”

“…This cut-oŠ value was set based on results of previous reports. [16][17][18][19][20] Statistical analysis We performed statistical analysis using statistical analysis software (StatView version 5.0, SAS, Cary, NC, USA); compared mean age, Child-Pugh score, PV pressure, PV-IVC pressure gradient, post-TIPS pressure gradient, or PV diameter between the 2 groups using unpaired t test; and compared sex, emergent case number, or therapeutic success, early mortality, onset or worsening of hepatic encephalopathy, technical di‹culty, or shunt dysfunction rates after TIPS using Fisher's exact probability test. Quantitative variables were expressed as mean±standard deviation (SD), and statistical signiˆcance was established as Pº0.05.…”
