Đặt vấn đề: Mở thông dạ dày được thực hiện khi cần nuôi dưỡng bệnh nhân qua đường tiêu hóa nhưng không ăn uống được bằng đường miệng. Ngày nay mở thông dạ dày hầu hết được thực hiện qua nội soi nhưng không phải trường hợp nào cũng thực hiện được. Để nuôi dưỡng cho những bệnh nhân ung thư thực quản cần hóa xạ trị tiền phẫu và sử dụng dạ dày thay thế thực quản khi tiến hành cắt bỏ thực quản sau xạ trị, chúng tôi đã thực hiện kỹ thuật mổ nội soi mở thông dạ dày bằng cách tạo ống thông từ mặt trước thân – đáy vị. Đối tượng và phương pháp nghiên cứu: Đây là một nghiên cứu can thiệp tiến cứu trên nhóm đối tượng bệnh nhân ung thư thực quản có chỉ định hóa xạ trị tiền phẫu và khối u lớn không thực hiện mở thông dạ dày được qua nội soi. Kết quả: Trong khoảng thời gian từ tháng 4 năm 2021 đến tháng 5 năm 2022 chúng tôi đã tiến hành phẫu thuật nội soi mở thông dạ dày cho 5 bệnh nhân ung thư thực quản một phần ba dưới giai đoạn bệnh cT3N0M0. Những bệnh nhân này có đặc điểm không ăn uống được bằng đường miệng do khối u lớn làm tắc nghẽn lòng thực quản. Kết quả phẫu thành công ở tất cả bệnh nhân, thời gian phẫu thuật từ 20 đến 45 phút. Không có trường hợp nào xảy ra tai biến và biến chứng. Bệnh nhân được cho ăn lại sau 24 giờ và chuyển khoa hóa xạ trị sau 3 đến 4 ngày để tiến hành hóa xạ trị tền phẫu. Kết luận: Phẫu thuật nội soi mở thông dạ dày bằng cách tạo ống thông từ mặt trước thân – đáy vị là một kỹ thuật an toàn, thời gian mổ nhanh và hồi phục sau mổ sớm.

Enteral nutrition, generally preferred to parenteral nutrition, is indicated when patients cannot meet their energy and metabolic demands. Gastrostomy tubes are placed directly into the stomach (either endoscopically, surgically, or radiologically) through the abdominal wall. Routine gastrostomy tube care is important to maintain well-functioning tubes. Postpyloric feeding tubes are preferable to gastric feeding tubes if patients have a history of aspiration, gastroesophageal reflux, severe gastroparesis, and/or recurrent nausea and vomiting. Feeding jejunostomy tubes are placed surgically and are indicated if gastric feeding is unsafe or impossible. Duallumen gastrojejunostomy tubes are used when both gastric decompression and feeding are desired. The general risks of enteral tube feeding include diarrhea, metabolic derangements, and aspiration. Additional complications for gastrostomy tubes, which can arise at any time, include tube dysfunction (clogging or deterioration), infection, bleeding, peristomal leakage, ulceration, gastric outlet obstruction, and accidental removal. After percutaneous endoscopic gastrostomy placement, there are also early or late complications that may occur. Multiple factors should be considered in the decision-making process for feeding tube placement. It is important to be realistic about the patient's prognosis and goals and to discuss the risks and benefits ahead of time. Consultation with palliative care or clinical ethics specialists should be considered in certain clinical scenarios.

“…If the radiologist and I do not think that there is a section of the stomach in close enough apposition to the anterior wall, then the case is referred for either surgical or radiological gastrostomy placement or some other approach such as a jejunostomy is entertained. In an exemplary study (2) from Missouri, the natural extension of my approach was used with the use of CT scan-guided gastrostomy tube placement in particularly challenging cases. Of 31 cases (about half of whom had had previous gastric surgery), only 1 patient suffered either of the above complications.…”
