CT differential diagnosis of COVID-19 and non-COVID-19 in symptomatic suspects: a practical scoring method

Abstract: Background: Although typical and atypical CT image findings of COVID-19 are reported in current studies, the CT image features of COVID-19 overlap with those of viral pneumonia and other respiratory diseases. Hence, it is difficult to make an exclusive diagnosis. Methods: Thirty confirmed cases of COVID-19 and forty-three cases of other aetiology or clinically confirmed non-COVID-19 in a general hospital were included. The clinical data including age, sex, exposure history, laboratory parameters and aetiologic… Show more

“…However, chest CT manifestations can vary in different patients and stages of infection, highlighting certain shortcomings of this approach. Apart from atypical manifestation that cannot be recognized by radiologists, several lung images are common in viral pneumonia leading to misdiagnosis [138]. Soon after the beginning of SARS-CoV-2 spreading, infected patients underwent antibody research for both basic research and clinical applications.…”

“…It is important to differentiate between reactive inflammatory changes close to the pleura and post-embolic reduced ventilation. Without contrast medium, however, perfusion changes cannot be detected [2][3][4][5][6][7].…”

“…A recently published study demonstrated that use of a simple scoring system based on seven common HRCT features (posterior part/lower lobe predilection, bilateral involvement, rounded GGO, subpleural bandlike GGO, crazy paving pattern, peripheral distribution, and GGO +/− consolidation) and four uncommon ones (single lobe involvement, central distribution, tree- in-bud pattern, and bronchial wall thickening) might be of help in categorizing symptomatic patients as COVID- 19 vs non−COVID-19. A high specificity (95.35 %) was achieved with a score greater than 4 [ 110 ].…”
