CT- and US-guided biopsy of the pancreas.

Abstract: A retrospective review of 211 computed tomographic (CT)-guided and 58 ultrasound (US)-guided biopsies of pancreatic lesions performed between 1985 and 1989 was undertaken to evaluate the accuracy of diagnosis and the number of complications. Combined CT and US accuracy in the diagnosis of malignancy was 93%. CT-guided biopsies had an accuracy of 86%, and US-guided biopsies had an accuracy of 95%. Accuracy was higher with larger masses (> 3.0 cm, 92%; < or = 3 cm, 81%) and larger needle sizes (16-19 gauge, 92%;… Show more

“…In many of these clinical scenarios, patients may un dergo percutaneous biopsy. The accuracy of biopsy for the diagnosis of cancer versus chronic pancreatitis varies, ranging from 45% to 94% [16,17].As a result, manypa tientsmay proceedto laparotomydespitethe associated costandmorbidity.…”

“…Whereas FNA only provides cells largely disrupted from their original arrangement, larger-caliber cutting needles allow for core biopsy specimens (11)(12)(13)(14)(15)(16)(17)(18). These specimens have been obtained by several routes (percutaneous, intraluminal and surgical) (16)(17)(18)(19)(20)(21)(22), and safety and accuracy of cutting biopsy have been previously demonstrated (16,(22)(23)(24).…”

“…In patients suffering from metastatic disease every effort is made to obtain a pathological diagnosis with percutaneous US-guided FNA, performing endoscopic palliation in patients with obstructive jaundice. In the vast majority of patients this approach can be considered the gold standard with the accuracy of percutaneous diagnostic procedures varying in the literature from 83 to 94% [24, 25]. …”
