CSF Dynamics: Implications for Hydrocephalus and Glymphatic Clearance

Abstract: Beyond its neuroprotective role, CSF functions to rid the brain of toxic waste products through glymphatic clearance. Disturbances in the circulation of CSF and glymphatic exchange are common among those experiencing HCP syndrome, which often results from SAH. Normally, the secretion of CSF follows a two-step process, including filtration of plasma followed by the introduction of ions, bicarbonate, and water. Arachnoid granulations are the main site of CSF absorption, although there are other influencing facto… Show more

“…Huge pituitary adenoma compresses the third ventricle before surgery leading to obstructive hydrocephalus, and it is easy to postoperative CSF leakage. Moreover, bleeding from the surgical area could cause widespread adhesion of the arachnoid membrane, resulting in CSF leakage or delayed CSF leakage ( 38 ).Besides, lower albumin levels of CSF may be a marker of the anti-inflammatory immune environment of invasive pituitary adenomas with higher infiltration of M2-like tumor-associated macrophages, which have an anti-inflammatory phenotype ( 39 ).…”

“…CSF circulating in the subarachnoid space drains into the subpial interstitial fluid (SPIF) from the perivascular spaces and exchanges substances with the BIF through the astroglial barrier or the blood through the capillary [51][52][53] (Figure 1E). Arachnoid granulations, which have been considered as the main pathway for absorbing CSF into venous sinus, may function as glymphatic-lymphatic coupling structures together with the newly unveiled subarachnoid lymphatic-like membrane (SLYM) [54][55][56]. The CSF-glymphatic communication through SLYM supervises the immune status of CSF and presents the information to the lymphatic and/or blood system through arachnoid granulations.…”
