Crushed Cartilage Grafts for Concealing Irregularities in Rhinoplasty

Abstract: The degree of crushing applied is important for long-term clinical outcome of autogenous crushed cartilage grafts. Slight or moderate crushing of cartilage creates an outstanding graft material for concealing irregularities and provides both excellent long-term clinical outcome and predictable esthetic results.

“…Once the costal cartilage grafts are fixed in place, any small irregularity on the surface of graft will appear on the skin once edema of the skin subsides especially in thin skinned patients. The edges of the dorsal only graft may show irregularities even after fine trimming [2]. Dorsal only diced cartilage wrapped in fascia have been used to mask dorsal irregularities.…”

“…In contrast, autologous fascia and diced cartilage, but not dermis (AlloDerm; LifeCell Corp), had been found to have low resorption rates in rabbits. 14 The degree of crushing of cartilage is known to have a strong effect on resorption rates, with intact or mildly crushed cartilage faring better in retrospective clinical 15 and animal 16 studies. Tutoplastprocessed fascia lata had been used successfully in combination with diced cartilage with high patient satisfaction rates and predictable outcomes for dorsal augmentation.…”

“…Camouflage of these irregularities represents one of the most challenging procedures of rhinoplasty. Various kinds of graft materials including autografts (cartilage, fascia and dermis), homografts (irradiated cartilage, preserved acellular dermis) and alloplasts (polyester, polyethylene) have been used to achieve better aesthetic results (Guerrerosantos, 1991;Sheen, 1998;Cakmak and Ergin, 2002;Sarukawa et al, 2004;Cakmak et al, 2005aCakmak et al, , 2005bBerghaus and Stelter, 2006;Ozaki et al, 2006;Cakmak and Buyuklu, 2007). Autogenous cartilage is considered to be ideal for all types of grafting because of its high survival rate, minimal resorption and low immunogenic characteristics.…”

“…In such cases, auricular or costal cartilage may provide an additional source of cartilage. Determinative factors to decide the donor site are based on the quantity and quality of cartilage needed, consent of the patient and experience of the surgeon (Horton and Matthews, 1992;A gao glu and Erol, 2000;Cakmak and Ergin, 2002;Cakmak et al, 2005aCakmak et al, , 2005bCakmak and Buyuklu, 2007).…”
