Critical phase distractions in anaesthesia and the sterile cockpit concept

Abstract: SummaryIn aviation, the sterile cockpit rule prohibits non-essential activities during critical phases of flight, takeoff and landing, phases analogous to induction of, and emergence from, anaesthesia. We studied distraction during 30 anaesthetic inductions, maintenances and emergences. Mean (SD) noise during emergence (58.3 (6.2) dB) was higher than during induction (46.4 (4.3) dB) and maintenance (52 (4.5) dB; p < 0.001). Sudden loud noises, greater than 70 dB, occurred more frequently at emergence (occurrin… Show more

“…He pointed to the difficulties in acquiring training and maintaining competence in the highly specialist field of pediatric/congenital TEE. He also drew attention to the “sterile cockpit” theory, which might suggest that the number of distractions (which includes the conduct of a TEE examination) should be kept to a minimum during cardiac anesthesia, concluding “You cannot do two complex things at once competently.”…”

“…There has been a great deal of study and research regarding distractions and how they can impact driving safety [21]. In a similar vein, distractions in the operating room can result in adverse outcomes for patients [22]. Law enforcement training programs and manuals address observational skills and factors such as stress which affect an officer's perception [23].…”

“…Miscommunication is one of the root causes of preventable medical errors because in an environment in which noise is present, verbal communication becomes more difficult, which in turn may lead to errors. 2,12,13 According to Broom et al, 14 environmental noise during critical phases of anesthesia can have a negative effect on the ability of the anesthesia professional to perform "nontechnical skills," such as maintaining situational awareness and effective communication. Further, the inability to maintain effective communication during critical anesthesia periods may result in appropriate patient care being "absent, misinformed, or misdirected and the opportunity for error increases."…”
