Critical Paths at Alliant Health System

Abstract: Alliant Health System (Alliant) was an enthusiastic, early user of critical paths in a hospital setting. In spite of its apparent early successes with the approach, Alliant eventually dropped critical paths as an overall corporate strategy and redesigned its approach to those critical paths still in use. This article summarizes the lessons learned and offers suggestions on the use of critical paths.

“…17 The format of care pathways varies from being a small-scaled paper-based document to elaborate computer programs that guide the healthcare professional through every step of the patient's management. 18 On the whole, care pathways are different from other forms of information provision systems that aim to assist healthcare professionals with clinical decision-making, such as guidelines and protocols. Furthermore, since care pathways are usually applicable to several aspects of care, they are also different from other mono-faceted tools such as diagnostic algorithm (for example, stroke diagnostic protocol for paramedical staff), or screening tools for specific interventions (for example, thrombolysis for acute ischaemic stroke).…”

“…After creating a suitable framework for the ambulatory wound pathway, a pilot phase of 3 months was initiated to verify clinical appropriateness and to determine interprofessional manageability. Piloting a pathway in a definite time period helps to identify areas in which the pathway may need to be changed and builds trust among hospital staff (11). During this phase, the modular pathway construction was partially rearranged due to emerging misunderstandings in the course of action and some apparent illogical catenations which solely came to the fore when the pathway was applied in practice.…”

“…15 Their impact may increase when and if the use of computerized systems for ordering laboratory tests, making referrals, and providing treatments and drugs becomes wideby guest on June 14, 2015 Health Affairs by content.healthaffairs.org Downloaded from spread in health care organizations, a development that will make it possible to compare physicians' decisions with guidelines in "real time." However, some people worry that guidelines could drive a wedge between physician and patient.…”
