Critical limb ischemia

Abstract: Patients with critical limb ischemia (CLI) present with ischemic rest pain or with tissue loss. Tissue loss, in its mildest presentation, includes focal ischemic ulceration or nonhealing wounds. Gangrene is the manifestation of severe chronic ischemia. Without revascularization, patients with CLI are at risk for limb loss and for potentially fatal complications from the progression of gangrene and the development of sepsis. The best patient outcomes are achieved when the diagnosis and appropriate treatment for… Show more

“…Pain in the muscles of the legs during walking, known as intermittent claudication, is the most common clinical presentation [2]. Left untreated, PAD can progress to chronic pain in the legs and eventually to non-healing wounds, gangrene, and limb loss [3]. Morbidity and mortality risk among PAD patients is high.…”

“…Subsequently, the blood flow in the periphery (in particular feet and legs) is reduced, leading to a chronically lowered tissue supply with nutrients and oxygen. This causes hypoxia and resulting critical limb ischemia (CLI), which is responsible for the degeneration of the affected skeletal muscles if left untreated for many years ( 3 , 10 ). Consequently, several gene therapy trials were established to improve microvascular supply in tissues of PAD patients ( 2 , 6 , 13 , 21 , 40 ).…”

“…Patients may present with prior conservative wound management and healed wounds (4345). Other patients can present and need an endovascular or surgical treatment in order to heal their wounds.…”
