This review summarises the role of microdialysis of the skin. It is a useful tool to evaluate the skin's biological processes in both healthy and inflamed skin. It can also enable a better understanding of the mechanism of action of certain drugs in inflamed skin. The microdialysis technique still needs to be standardised so that its practice can be used in a reproducible fashion in clinical studies. It would be interesting to carry out microdialysis in subjects with the skin condition, hidradenitis suppurativa as this research has not yet been conducted. Microdialysis is a minimally invasive technique to study metabolic, biochemical, and pharmacological events in tissue. Factors that influence microdialysate collection include molecular weight cutoff of the membrane, perfusion rate, perfusate viscosity, duration of collection, depth of the catheter, length of the tubing and adsorption of hydrophobic molecules to the membrane. To standardize these factors, a robust sampling protocol needs to be established. Microdialysis is applied in healthy and inflamed skin. It enables the in vivo sampling of endogenous and exogenous substances in skin's extracellular fluid. In atopic dermatitis, levels of neuropeptides, eicosanoids and histamine pre-and post-treatment treatment have been conducted. Microdialysis in atopic skin has assessed the pharmodynamics of a number of topical drugs. In psoriatic skin, the 'cytokine fingerprint' has been evaluated through microdialysis and bioassays. This unique fingerprint has also been analyzed after certain pharmacological treatments for psoriasis.

Diphenhydramine hydrochloride (DPH) is an antihistamine used topically to relieve skin itching and, at higher doses, DPH also has the capability to induce local skin anesthesia. The present study investigates the possibility to deliver DPH via iontophoresis using a stainless steel electrode. In-vitro iontophoresis was performed using 1% (w/v) DPH dissolved in glycerin (100%), deionized (DI) water (100%) and a mixture of both (50:50) through porcine ear skin and Franz cell assembly. The applied electrical current was 0, 100, 200, and 300 µA/cm 2 for 60 min. Of the solvents tested, the 100% glycerin provided a consistent increase in DPH skin concentrations with current density and no skin damage. Therefore, the 100% glycerin was selected for in-vivo iontophoresis in a rabbit model at the same three current densities for 60 min. DPH skin concentrations were measured in skin via microdialysis for 240 min. Plasma to skin distribution of DPH was studied following short IVinfusions of 6, 8 and 10 mg/Kg in rabbits. Skin and plasma samples were collected for 420 min. No skin damage was observed even at the higher current density. DPH skin concentrations were higher following iontophoresis than IV-infusion. Thus, iontophoresis of DPH in glycerin from a stainless steel patch may be a safe, effective, and inexpensive way to deliver high dose of DPH topically to the skin.

Recent advances and development in epidermal and dermal drug deposition enhancement technology

Skin is the largest and easily accessible organ of the body. Increases in incidences of dermatological disorders, demand for drug targeting, and patient compliance have increased the popularity of topical drug delivery amongst the people. However, drug delivery across the skin is still a challenge for researchers because permeation of maximum drugs is hindered by the upper layer of the epidermis (stratum corneum). Several approaches like use of chemical permeation enhancers and physical methods such as sonophoresis, iontophoresis, electroporation, microneedles, etc., have been used to deliver the drugs topically. These methods of topical drug delivery have some limitations and drawbacks. Therefore new techniques based on nano drug delivery system such as ultradeformable liposomes, nanostructured lipid carriers, nanoemulsions, solid lipid nanoparticles, lipospheres, nanoparticles, and ethosomes have been exploited for enhancing epidermal and dermal drug deposition. Development of these nanosytems requires a good understanding of mechanism of drug permeation, physicochemical properties of drug and carriers, and technological advancements in methodology. Therefore, this article covers recent advances in epidermal and dermal drug deposition enhancement approaches, biopharmaceutical challenges with dermal drug delivery, issues in formulation development, and regulatory aspects of nanosystem. This review article also discusses the concern of topical drug delivery in immunization, gene delivery, and cosmeceuticals.
