Crimean–Congo hemorrhagic fever: a first case from Abbottabad, Pakistan

Abstract: Crimean-Congo hemorrhagic fever (CCHF) is a deadly viral disease that is endemic in some parts of Pakistan. We report here the first ever case of CCHF from Abbottabad. The patient presented with abdominal pain, hematemesis and low platelets, and died within 24hours of admission. The diagnosis was confirmed by PCR for the CCHF virus. Because of the aggressive infection-control measures adopted, secondary and nosocomial spread was prevented.

“…25 Human-to-human transmission occurs by infected blood or secretions but airborne transmission of the disease has not been documented. 8 All three of our nosocomial cases were involved in high-risk procedures such as inserting nasogastric and endotracheal tubes, with splashing of blood into their faces, percutaneous contact with patients' blood (intact skin), and cutaneous contact with non-sanguineous body fluids. These findings support the use of contact and droplet isolation as precautions for prevention of transmission.…”

“…9 Prophylaxis is suggested after high-risk contamination. 8 In our experience, ribavirin is potentially useful and should therefore be recommended for healthcare workers who are at risk of exposure, such as by percutaneous injuries. None of our cases received prophylaxis.…”

“…CCHF is a zoonosis (by wild and domestic animals) and widespread tick-borne(genus Hyalommafamily: Ixodidae). Due to vertical or transovarian transmission of hereditary has been preserve in the life cycle ticks, which helps keep to the viral reservoir in arthropod [2]. CCHF can cause severe infection in humans with a fatality rate of up to 25%-30%.…”
