Coxsackie Eruption Arising in Areas of Epidermolytic Ichthyosis

Abstract: Coxsackie eruptions concentrated in areas of atopic dermatitis, a phenomenon termed "eczema coxsackium," has been well described in the literature but, to our knowledge, the concentration of coxsackie viral lesions to areas of ichthyosis has not been reported. This case describes a rare presentation of coxsackie viral lesions concentrated in areas of epidermolytic ichthyosis in a 3-year-old boy.

“…They also noted a tendency for localization to sites of skin injury or inflammation, such as sunburns, irritant/diaper dermatitis, healing lacerations, and tinea pedis. 3 Since then, multiple cases of EC have been reported in both children and adults; however, to our knowledge, besides one report of coxsackievirus A9 developing within lesions of mosaic epidermal ichthyosis, 5 no serotypes other than CVA6 and CVA16 have been reported to cause EC. 6 …”

“…The reason why enteroviruses appear in AD lesions is still unclear. One hypothesis suggests that the mechanism is similar to that of EH, in which afflicted patients appear to have impaired immunity to the virus and barrier dysfunction at affected sites 111 . This hypothesis may explain why a widespread viral infection can also erupt at noneczematous skin sites, such as in Darier disease 112 …”
