Objective To the effectiveness of different strategies to overcome silver diamine fluoride (SDF)‐mediated tooth staining. Materials and Methods Four online databases (PubMed, ScienceDirect, Scopus, and Web of Science) were searched using different MeSH terms and Boolean Operators to retrieve the articles (until June 2021), followed by a hand‐search of the reference list of the included articles. All full‐text, original studies in English that evaluated SDF staining and at least one SDF modification/alternative were included. Results Among the assessed studies, nine studies explored the stain‐minimization effect of potassium iodide (KI) post‐application following SDF treatment. Among these, eight concluded that KI application after SDF treatment significantly reduced tooth staining, while one showed marginal staining following glass ionomer restoration of the SDF‐treated dentine. Additionally, one study applied potassium fluoride (KF) and silver nitrate (AgNO3) concurrently to mitigate SDF‐mediated staining. One study compared SDF staining with polyethylene glycol (PEG)‐coated nanoparticles containing sodium fluoride (NaF), and another used nanosilver fluoride (NSF) for staining comparison with SDF. Conclusions Within the limitations of this study, the addition of different materials to SDF has proven to be a beneficial strategy for overcoming tooth staining associated with SDF. Future studies are warranted, particularly clinical trials, to validate these findings. Clinical Significance SDF‐mediated tooth staining is a serious concern that limits its clinical use. A review of various strategies to overcome this problem will help clinicians enhance its clinical use and patient acceptance.

Parental Views on Minimally Invasive Dentistry versus General Anaesthesia Extractions of Children’s Compromised First Permanent Molars: An Exploratory Qualitative Study

Introduction: The current standard of care for first permanent molars (FPMs) requiring extraction is removal of these teeth between the chronological ages of eight to ten years, as per UK guidelines.1 This often involves a general anaesthetic (GA) with surgical admission to hospital. This study explores parental views on minimally invasive (MI) techniques as an alternative to the UK current standard of care for extractions of FPMs deemed to require removal between the chronological ages of eight to ten years. Methods: A qualitative investigation, using semi-structured interviews was conducted with parents/carers of children attending a teaching hospital for extraction of compromised FPMs under GA. Thematic framework analysis was used to present the findings. The consolidated criteria for reporting qualitative research (COREQ) were used as a guide to ensure quality. Results: The main themes emerging were: participants’ surprise at how poor the prognosis was for their child’s FPMs; acceptance that care was beyond the scope of primary care; willingness by some to undergo GA again; requests for information about the guarantee of success of MI treatment; concerns about the residual black staining from silver diamine fluoride (SDF); and acceptance of extraction spaces because of potential future failure of MI. Conclusion: This exploratory qualitative study has shown that, while extraction of compromised FPMs under GA is accepted by most parents/carers, there appears to be a growing acceptance of MI approaches to restore FPMs instead of extraction of these teeth.

ÖZET Geçmişten günümüze birçok farklı meslekte ve sağlık sektöründe kullanılmakta olan gümüşün diş hekimliğindeki endikasyonları ve popülerliği giderek artmaktadır. Sahip olduğu antibakteriyel özelliği, karyojenik bakteriler üzerindeki bakteriostatik ve bakterisidal etkinliği çürük tedavisindeki kullanımını desteklemektedir. Florür ile bir arada kullanımı sinerjitik etki oluşturabilmektedir. İdeal özellikte bir ürün geliştirebilmek için farklı içeriklerde, kombinasyonlarda ve miktarlarda gümüş bulunduran materyallerle ilgili çok sayıda çalışma ve araştırma yapılmaktadır. Gümüş ve florür iyonlarını bir arada bulunduran gümüş florür ve gümüş diamin florür (GDF) içerikli ticari ürünler piyasada mevcuttur. Dünya Sağlık Örgütü, küçük çocuklarda oldukça sık görülebilen erken çocukluk çağı çürüklerinin tedavisinde GDF solüsyonunun kullanımını desteklediklerini belirterek, hem erişkinler hem de çocuklar için ilaç kategorisine aldıklarını açıklamışlardır. Ancak bu solüsyonların diş dokusunda siyah renklenmeye neden olabilmesi potasyum iyodür ve glutatyon gibi bileşiklerle birlikte kullanımını gündeme getirmiştir. Son dönemde nanoteknolojideki gelişmeler ışığında nanopartikül boyutunda gümüş bulundurduğu için renklenmeye neden olmadığı ve yüksek antibakteriyel etkinliğe sahip olduğu belirtilen ve deneysel bir solüsyon olan nanogümüş florürün standardizasyonunu sağlayabilmek için çalışmalar devam etmektedir. Its antibacterial properties, bacteriostatic and bactericidal activity on cariogenic bacteria support its use in caries treatment. It can create a synergistic effect when used together with fluoride. In order to develop an ideal product, many studies and researches are carried out on materials containing silver in different contents, combinations and amounts. Commercial products containing silver fluoride and silver diamine fluoride (SDF), which contain silver and fluoride ions, are commercially available. The World Health Organization stated that they support the use of SDF solution in the treatment of early childhood caries, which can be seen quite frequently in younger children, and announced that they included it in the category of drugs for both adults and children. However, the fact that these solutions can cause black discoloration in the dental tissue has led to the use of compounds such as potassium iodide and glutathione.
