COVID-19 and Acute Pancreatitis: A Systematic Review of Case Reports and Case Series

Abstract: BACKGROUND: Coronavirus disease 2019 (COVID-19) presents mainly with mild symptoms and involvement of the respiratory system. Acute pancreatitis has also been reported during the course of COVID-19. OBJECTIVE: Our aim is to review and analyze all reported cases of COVID-19 associated acute pancreatitis, reporting the demographics, clinical characteristics, laboratory and imaging findings, comorbidities and outcomes. “…In the present study, liver biomarkers were associated with patient outcomes. It is known that SARS-CoV-2 is a ------------------------------------------------------------- systemic infection that affects numerous organs, including the kidneys, pancreas, liver and heart (20)(21)(22)(23). Liver impairment is present in more than half of individuals infected with SARS-CoV-2, and this impairment becomes more severe as the disease progresses.…”

“…This observation might at least partially explain the ability of SARS-CoV to continuously destroy the lung tissue, which is associated with limited repair capacity. The fact that ACE2 is also expressed in the heart, renal, and intestinal tissues [ 49 ] may also at least partially explain the multi-organ injury of SARS-CoV2-infected patients [ 50 , 51 , 52 , 53 ].…”

“…In imaging studies, computed tomography and magnetic resonance imaging, abdominal findings were compatible with acute interstitial edematous pancreatitis in these patients. This acute abdomen condition, in addition to being related to the SARS-CoV-2 infection, was the result of the late search for hospital care, which also helped to increase the conversion rate to open surgery and the number of perforating conditions [6][7][8][9][10][11][12] .…”

“…The expression of ACE2 receptors can lead to damage to pancreatic cells, hepatocytes, gallbladder mucosa, and proximal and distal enterocytes of the small intestine, with greater expression in the brush border of intestinal enterocytes and in absorptive enterocytes of the ileum and colon than in the lung. The direct cytotoxic action of SARS-CoV-2 or the indirect systemic inflammation mediated by the immune system may be the pathogenesis mechanisms of these lesions 2,11,12,15 .…”
