Course of Depressive Symptoms in Patients Undergoing Heart Surgery: Confirmatory Analysis of the Factor Pattern and Latent Mean Structure of the Center for Epidemiologic Studies Depression Scale

Abstract: Objective: The purpose of this study was to examine the dimensionality, stability, and course of depressive symptoms over the 12-month period beginning approximately 1 week before heart surgery. Methods: The Center for Epidemiological Studies Depression Scale (CES-D) was administered to 570 patients before heart surgery and 1, 3.5, 6.5, and 12.5 months later. Results: Confirmatory factor analysis rejected a four-factor model as a result of small variances for two interpersonal items. With their elimination, a … Show more

“…It has a stable factor structure across large clinical and nonclinical samples, high internal and adequate test-retest reliability, and good convergent and discriminant validity (Contrada et al, 2006; Radloff, 1977). It has strong psychometric properties in older populations (Herzog et al, 1990).…”

“…The NHFA guidelines emphasise repeated depression assessments to identify changing depression trajectories after a CHD event. Evidence documents a number of post‐CHD event patterns including spontaneous remission (Contrada, Boulifard, Idler, Krause, & Labouvie, ; Murphy et al., ; Wade et al., ). Watchful waiting, monitoring, and brief support of suspected depressed cases might serve as a useful strategy before implementing psychological intervention in some patients with lower depression scores (Lichtman et al., ).…”

“…As previously mentioned, the somatic laden, depression diagnostic criteria overlap CAD symptoms. Specifically, fatigue, loss of appetite, psychomotor retardation, insomnia, and difficulty concentrating can be the direct physiological response to a medical illness and hospitalization[36] and have been documented to significantly increase in the first month after CABG surgery [37]. Brief psychological reactions to the impending stressors of surgery and the post-operative recovery period may spontaneously remit over time, thus requiring no further intervention.…”
