Course and outcome of first-admitted patients with acute and transient psychotic disorders (ICD-10:F23)

Abstract: Only a minority of first-hospitalized patients with ATPD develop a severe social impairment after three to seven years. This subgroup, however, is not compatible with the concept of a "transient" psychotic disturbance, but rather with an early manifestation of a chronic schizophrenic disorder.

“…In other words, the proposal to reduce ATPD to the APPD category alone may be premature (Nugent et al, 2011). Our results are in line with earlier research suggesting that APPD is a heterogeneous diagnostic category with variable outcomes (Jager et al, 2003;Abe et al, 2006) and highlight the need for larger scale, prospective studies of the acute psychotic disorders before meaningful changes to their classification can be made.…”

“…Third, the lack of standardized psychometric assessment scales for measuring the severity of baseline psychotic features was also a limitation. Some studies have reported that FEP patients with ATPD showed higher scores in anxiety and excitement and fewer negative symptoms than those who later transitioned to schizophrenia . However, it should be noted that other authors have not found differences in terms of original symptom severity .…”

“…[79] Marneros et al . [510] reported that 75% of their cases with ATPD had a recurrent affective or psychotic episode, 30% developed affective disorders, and a relatively small number developed schizoaffective disorder or schizophrenia.…”
