County-level outcomes of a rural breast cancer screening outreach strategy: a decentralized hub-and-spoke model (BSPAN2)

Abstract: Rural mammography screening remains suboptimal despite reimbursement programs for uninsured women. Networks linking non-clinical community organizations and clinical providers may overcome limited delivery infrastructure in rural areas. Little is known about how networks expand their service area. To evaluate a hub-andspoke model to expand mammography services to 17 rural counties by assessing county-level delivery and local stakeholder conduct of outreach activities. “…Consistent with findings in more established cancer screening programs ( Burnett-Hartman et al, 2016 ; Lee et al, 2017 ; Peppercorn et al, 2015 ), differences exist for lung cancer screening by race and geographic area of residence. “…Providing the economic resources needed to support facilities and planning for sustainability of a project is key. Establishing monitoring and evaluation systems, along with the incorporation of goals that span from immediate to long-term, has been shown to help with successful multilevel implementation efforts (34). External community partners from domains outside of cancer control and health care, such as social services, housing, faith-based organizations, and community development, are needed for interventions to scale up in rural areas.…”

“…The adoption of innovations is improved when devoted individuals, often referred to as champions, facilitate implementation [ 3 , 12 , 13 ]. Champions are individuals (health care providers, management [ 14 , 15 ], or lay persons [ 16 , 17 ]) who volunteer or are appointed to enthusiastically promote and facilitate implementation of an innovation [ 13 , 18 , 19 ]. There is confusion and overlap between the concept of champion and other concepts, such as opinion leaders, facilitators, linking agents, change agents [ 19 , 20 ], coaches and knowledge brokers [ 19 ].…”
