Costs of accessing HIV testing services among rural Malawi communities

Abstract: HIV testing is free in Malawi, but users may still incur costs that can deter or delay them accessing these services. We sought to identify and quantify these costs among HIV testing service clients in Malawi. We asked residents of communities participating in a cluster randomised trial investigating the impact of HIV self-testing about their past HIV testing experiences and the direct non-medical and indirect costs incurred to access HIV testing. We recruited 749 participants whose most recent HIV test was wi… Show more

“…Additionally, our costs were comparable to one study in Côte d'Ivoire reporting HTS unit costs from the Ivorian Programme National de Lutte contre le Sida (PNLS) for FSW and MSM at $16 and $21, respectively (41). However, one should consider the reduced costs to the kit user (in terms of transportation cost or opportunity cost for example), and therefore to society, when comparing community-based HIVST distribution and facilitybased provider-delivered HTS costs (42,43).…”

“…A study in these same communities in Malawi showed the mean costs of accessing HIV testing among women and men as US$1.83 and US$3.81, respectively, with men reporting significantly higher opportunity costs (i.e. lost income) . Community HIVST distribution reduces these costs to nearly zero, as kits are delivered in the home with no waiting times.…”

“…Further studies tested user preferences and established simple distribution models aimed at using HIVST for maximum public health impact . Data from these early distribution models informed costing and cost‐effectiveness studies to establish the evidence for HIVST scalability and sustainability . This collaboration helped generate important aspects of the evidence needed to inform WHO Guidelines on HIVST and Partner Notification (i.e., HIVST normative guidelines) published in December 2016, and the operational guidance at country level that followed.…”
