Costly comorbidities of COPD: the ignored side of the coin?

Abstract: Correct, the alliteration in this editorial's title is not a co-incidence! Ironically, COPD and comorbidity both start with two identical letters and previously in this journal it has even been suggested twice that COPD could also stand for "COmorbidity with Pulmonary Disease" [1, 2]. However, after having been the only notorious couple for a long time, from now on they will have to face some fierce competition from a new rising kid on the "co"-block: costs.

“…These results, in the context of a relatively short recall period, would indicate telescoping of visits (forward or backwards) rather than memory decay as the source of inaccuracy. Additionally, less accurate self-report of healthcare utilisation by participants with a higher COTE index was demonstrated, which is an important consideration in people with COPD who frequently demonstrate comorbidities that have considerable clinical and economic consequences [ 35 ].…”

“…While our study was not powered to assess mortality, all-cause, cardiac-and respiratory-related deaths were all lower in the self-management group, providing some evidence that multimodal action plans with ongoing case-manager support are safe. Forthcoming cost analyses will assess the cost-effectiveness of the self-management intervention as the majority of costs in COPD patients with comorbidities are related to treatment of exacerbations, with hospitalisation costs a major component [4,26].…”

“…There are shared pathophysiological mechanisms in COPD and other chronic diseases. For example, some comorbidities may have a substantial impact on health and healthcare services use, leading to increased severity and hospitalization of COPD patients and some others such as heart disease can lead to death earlier than respiratory causes 13 .…”
