Cost Utility of Schlemm’s Canal Microstent Injection With Cataract Surgery for Open-angle Glaucoma in the US Medicare System

Abstract: Précis: Hydrus microstent (HMS) implantation at the time of cataract surgery appears to be cost-effective in mild-to-moderate glaucoma. However, long-term follow-up is essential for a full assessment of device performance, safety and cost-effectiveness. Purpose: The aim was to assess the societal cost-utility to the US Medicare system of implanting HMS with cataract surgery versus cataract surgery alone in patients with open-angle glaucoma. Patients: Markov model cohort of patients with mild-to-moderate op… Show more

“… 34 Despite the increasing use of MIGS in clinical practice, data to compare the cost-effectiveness of these devices remain limited. 43 Separate studies have shown either the Hydrus 44 or the iStent inject 45 46 combined with phacoemulsification is cost-effective for patients with mild to moderate OAG versus phacoemulsification alone. A recent study by Sood et al 47 compared the cost-effectiveness of the Hydrus and the iStent inject in combination with phacoemulsification for patients with mild to moderate primary OAG.…”

“…1 Indeed, our paper focused on direct costs associated with Hydrus microstent (HMS) implantation during cataract surgery in open angle glaucoma. 2 We appreciate that glaucoma has a significant indirect economic burden on society and that these are also important to measure. 3 However, economic modeling is complex and incorporating indirect costs into an already complicated model was out of the scope of our study.…”

“…We have recently read with great interest a paper by Atik and colleagues1,2 published in your journal. The authors used a cost-effectiveness analysis to prove that Schlemm canal microstent injection with cataract surgery appeared to be a more cost-effective strategy over single cataract surgery with following medication in the management of mild-to-moderate open angle glaucoma.…”
