Abstract:Shorter operative time and day-case admission for MIP generate costs savings that compensate only partially for the additional costs associated with parathyroid imaging studies.
“…This proportion is twice that observed in the national audit organized by the British Association of Endocrine and Thyroid Surgeons 4 . As expected, MIP is associated with a significantly shorter operating time and a higher proportion of patients being treated as a day case, 28 particularly those aged below 60 and to a lesser extent those aged 60–75.…”
Parathyroidectomy is safe in the elderly and is associated with a significant improvement in symptoms. As survival after operation is similar to younger patients, surgery should be considered in all elderly patients with PHPT.

“…The cost effectiveness of localization procedures is controversial [3,35]. In the present study, sestamibi scintigraphy was TP for a solitary adenoma in 64% of the patients and in 58% of the patients for ultrasound.…”
Localization procedures and iOPTH decreased the risk for hypocalcemia after pHPT surgery. Additionally, iOPTH influenced short-term cure rate favorably.
“…In a very large prospective cohort study of 3,000 consecutive patients undergoing parathyroidectomy and discharged within 2.5 h after the operation, only six patients (0.2%) required a visit to the emergency department for intravenous calcium infusion, all occurring on postoperative day3 or later, and none of these patients required rehospitalization [29]. In the experience at Oxford, 74 of 150 patients were discharged the same day and 70 patients were discharged within 23 h after SP [15], while 71 patients undergoing BNE spent a total of 93 days in hospital and only 66% of them were discharged within 24 h [30].…”
Selective parathyroidectomy is an option for patients with positive preoperative localization tests undergoing first-time surgery who have no family history of pHPT, no goiter for which surgical therapy is proposed, and are not on lithium therapy.
scite is a Brooklyn-based organization that helps researchers better discover and understand research articles through Smart Citations–citations that display the context of the citation and describe whether the article provides supporting or contrasting evidence. scite is used by students and researchers from around the world and is funded in part by the National Science Foundation and the National Institute on Drug Abuse of the National Institutes of Health.
