Cost-effectiveness analysis of biopsy strategies for endometrial cancer diagnosis in women with postmenopausal bleeding: Pipelle sampling curette versus dilatation & curettage

Abstract: The Pipelle is the more cost-effective sampling strategy compared to D&C for EC diagnosis in women with PMB. From the cost-effectiveness perspective, the higher sampling failure rate of Pipelle should not be regarded as a limitation in its clinical application.

“…No difference in the incidence of coexisting cancer at hysterectomy was observed between the two methods (41% vs 45%; P > 0.05), but this study conclusion was limited by the small sample size. Another study comparing diagnostic accuracy between Pipelle endometrial sampling and D&C in women with postmenopausal bleeding presented that the Pipelle strategy has not only similar effect, but also has more cost-effective compared to the D&C [11]. The costeffectiveness may be considered as an important part of clinical decision making between aspiration biopsy and D&C procedure.…”

“…The traditional diagnostic methods are diagnostic curettage or hysteroscopy, but both are invasive and cannot accurately reflect the breadth and depth of the lesion ( 11 ). Although MRI can be used clinically to distinguish benign from malignant endometrial lesions and assess the degree of invasion ( 1 ), it is not widely available clinically due to its high price and long examination time.…”

“…Endometrial biopsy with an aspirational catheter is an outpatient procedure that may have less diagnostic value because only tiny fragments of tissue are removed and therefore a significant number of missed lesions are to be expected. A more recent diagnostic method for uterine mucosa sampling is hysteroscopic biopsy, which is now considered the gold standard in uterine cancer diagnostics ( 8 ). The method is also more sensitive for detecting benign pathologies such as endometrial polyps or submucosal myoma ( 5 , 7 ).…”
