“…If the difference is greater in bright lighting conditions, as in this case, the larger pupil is abnormal. The main differential diagnosis to consider for a large pupil is a third nerve palsy, pharmacological mydriasis (including cosmetic and botanic mydriasis),3–5 tonic pupil and local iris processes, such as iris sphincter tears from trauma. Slit-lamp examination will reveal any signs of trauma or inflammation, which were not present in this case.…”
