Introduction Cervicogenic headache (CH) is a chronic headache that is referred to the head from the cervical spine that typically starts or worsens with neck movement. The pain typically presents posteriorly with anterior radiation. Nociceptive input from structures innervated by the upper three cervical spinal nerves can theoretically exacerbate primary headache disorders like migraine. We present three cases of CH related to C1-C2 joint arthropathy that significantly improved after treatment with calcitonin gene-related peptide (CGRP) antibodies initiated for chronic comorbid migraine. Case Descriptions We describe a case series of 3 patients with co-morbid CH and migraine that were treated with CGRP antibodies. All three patients had significant improvement in their migraines with C1-C2 joint injections and subsequently CGRP antibodies provided a greater than 50% benefit in each patient preventing the need for repeat injections. Conclusions The diagnosis of CH in patients is confirmed when treatment of the cervical spine lesion alleviates pain significantly. While CH itself has not been shown to be associated with elevated serum CGRP as has been demonstrated in migraine, animal models of osteoarthritis have shown reduction of hyperalgesia with galcanezumab. An upregulation in CGRP related to nociceptive input from degenerative changes in the cervical spine could lead to potentiation and sensitization which could potentially respond to systemic administration of CGRP antibodies. As a result, the presence of features suggestive of CH in patients with migraine may be a predictor of a positive response to CGRP antibodies. For patients with comorbid migraine and CH, CGRP antibodies could prevent the need for invasive injection treatments.

The aim of preventive treatment of episodic and chronic migraine is to reduce the frequency and severity of seizures and thereby improve the quality of patients' lives. It is estimated that 38% of patients with migraine would benefit from preventive treatment. However, in clinical practice only approximately 13% of patients with migraine receive it. Intolerable side effects and lack of efficacy of pharmacological treatment are the main reasons of treatment discontinuation. It is suggested that calcitonin-gene-related-peptide (CGRP) plays a critical role in migraine pathophysiology. One of them is galcanezumab – a humanised monoclonal antibody that selectively binds to the CGRP peptide. Galcanezumab has demonstrated efficacy in EVOLVE-1, EVOLVE-2 and REGAIN trials. It significantly reduces migraine days per month versus placebo. Moreover, the quality of patients' lives treated with galcanezumab also improved. It has a favourable safety profile, similar to placebo groups. The most frequent adverse event in patients treated with galcanezumab was injection site pain. No toxic effects on the liver or negative influences on hemodynamic or laboratory parameters were reported. The discontinuation rates due to adverse events in patients treated with galcanezumab were low. In October 2019, lasmiditan was approved by FDA and EMA in prophylaxis treatment of migraine in adult patients. Currently, this medicine is not accessible in Poland.

Migraine is one of top 5 medical conditions that contribute to Years Lived with Disability and affects approximately 1 billion people from around the world. To date, preventive treatment and acute therapies for migraine are limited, have undesirable side effects and are poorly tolerated in patients. In the last few decades, considerable advances in our understanding of migraine and its pathophysiology have paved the way for the development of targeted treatment options. Calcitonin gene-related peptide (CGRP) plays an integral role in the neurobiology of migraine, and new classes of drugs that target the CGRP pathway have included gepants and CGRP pathway monoclonal antibodies. Serotonin 5-HT1F receptor agonists—namely ditans—have also been developed to treat acute migraine. Lastly, non-invasive neuromodulation offers another treatment option for migraine patients who prefer treatments that have fewer side effects and are well tolerated. In this review, we discussed emerging treatment options for migraine that were made available in recent years.
