Objective. This research was to detect the treatment rate and prognosis of elderly patients with acute stroke in emergency department by the optimization of emergency care applying meta-analysis. Methods. The online databases including PubMed, EMBASE, ScienceDirect, Cochrane Library, China knowledge Network Database (CNKI), China VIP Database, Wanfang Database, and China Biomedical Literature Database (CBM) were searched. The retrieval time limit was set from the establishment of the database to the present. The data were extracted independently by two investigators. The bias of per publication was assessed following Cochrane manual 5.1.0 standard. RevMan5.4 statistical software was used to analyze the collected data by meta. Results. The 8 randomized controlled trials included in this meta-analysis all reported patients’ baseline status. The meta-analysis of the rescue time of the study group and the control group ( CH 2 = 1507.80 , df = 4 , P ≤ 0.001 , and I 2 = 100 % ) showed that the emergency nursing with optimized emergency procedures can shorten the rescue of elderly acute stroke patients in the emergency department time. There are 6 literatures reporting the case fatality rate ( Ch i 2 = 1.12 , df = 5 , P = 0.95 > 0.05 , and I 2 = 0 % ), and the death rate of the study group was not higher than that of the control group ( Z = 4.4 and P < 0.0001 ). The use of optimized first aid can reduce mortality in elderly patients with acute stroke. Six articles on disability rate reported the heterogeneity of disability rate ( CH 2 = 2.88 , df = 5 , P = 0.72 > 0.05 , and I 2 = 0 % ), indicating that the disability rate in the study group was lower than that in the control group ( Z = 3.91 and P < 0.0001 ), indicating that emergency nursing by optimizing emergency procedures can reduce the disability rate of elderly stroke patients in emergency department. Conclusion. Optimizing the emergency care process can effectively improve the emergency rate and prognosis of elderly patients with acute stroke in emergency department; however, further research with higher methodological quality and longer intervention time are needed to verify later.

Objective Due to variability in reports, the aim of this meta-analysis was to evaluate the incidence and risk factors of post-stroke early seizures (ES) and post-stroke epilepsy (PSE). Methods The MEDLINE, EMBASE and Web of Science databases were searched for post-stroke ES/PSE articles published on any date up to November 2020. Post-stroke ES included seizures occurring within 7 days of stroke, and PSE included at least one unprovoked seizure. Using random effects models, the incidence and risk factors of post-stroke ES and PSE were evaluated. The study was retrospectively registered with INPLASY (INPLASY2023100008). Results Of 128 included studies in total, the incidence of post-stroke ES was 0.07 (95% confidence interval [CI] 0.05, 0.10) and PSE was 0.10 (95% CI 0.08, 0.13). The rates were higher in children than adults. Risk factors for post-stroke ES included hemorrhagic stroke (odds ratio [OR] 2.14, 95% CI 1.44, 3.18), severe strokes (OR 2.68, 95% CI 1.73, 4.14), cortical involvement (OR 3.09, 95% CI 2.11, 4.51) and hemorrhagic transformation (OR 2.70, 95% CI 1.58, 4.60). Risk factors for PSE included severe strokes (OR 4.92, 95% CI 3.43, 7.06), cortical involvement (OR 3.20, 95% CI 2.13, 4.81), anterior circulation infarcts (OR 3.28, 95% CI 1.34, 8.03), hemorrhagic transformation (OR 2.81, 95% CI 1.25, 6.30) and post-stroke ES (OR 7.24, 95% CI 3.73, 14.06). Conclusion Understanding the risk factors of post-stroke ES/PSE may identify high-risk individuals who might benefit from prophylactic treatment.

Approximately 69 million people worldwide are annually affected by traumatic brain injury (TBI). In Malaysia, the traumatic injury was the leading cause of hospital admission and death, accounting for one in three emergency visits. Among the most recognised complication of TBI is post-traumatic epilepsy (PTE), which is an essential contributor to morbidity and mortality. However, there is a lack of local epidemiological data on PTE in Malaysia. This study aims to describe the incidence and predictors of PTE among TBI patients admitted to a tertiary healthcare centre in Kuala Lumpur, Malaysia. We hypothesised that increases in age, race, and severity of brain injury are among the main potential predictors of PTE. It will also provide evidence that patients with epilepsy following TBI are associated with significant impairment in cognitive performance than TBI patients without epilepsy. An analysis of a two years retrospective cohort will be employed, of which adults with a history of admission for TBI in 2019 and 2020 will be contacted, and the development of epilepsy will be ascertained using a validated tool and confirmed by our neurologists during visits. The patients will then be grouped into two, with PTE and without PTE, and assessed their cognitive performance by clinical psychologists. Given that the management of TBI and PTE patients involves a multidisciplinary team, the findings might be significant to many healthcare providers in determining policy and strategise a better treatment.
