A systematic review of current data on the structure and functions of Helicobacter pylori (Hp), pathogenesis, diagnosis and treatment of Hp-infection is presented. The relevance of the topic of helicobacteriosis is due to the proven connection of this infection with chronic, widespread diseases of the digestive organs among the population. The review used materials from the databases including PubМed, Medline, EMBASE, Scopus, Google Scholar, RSCI. A systematic review based on studies with a high degree of evidence has shown significant progress in the diagnosis and treatment of Hp-infection in recent years in adults and children. Coordination of the scientific search for the regularities of the formation of Hp-associated forms of pathology, continuous improvement of diagnostic technologies and the search for new therapies, periodic well-organized international revisions of standard methods of Hp diagnosis and treatment contribute to the creation of effective methods of Hp eradication, preservation of the optimal composition of the microbiota of the gastrointestinal tract and prevention of antibiotic resistance. Of particular importance for the prevention of complications of Hp-infection are original diagnostic algorithms using specialized endoscopic and molecular technologies before and after Hp eradication with long-term follow-up and the determination of the leading factors correlating with a high risk of stomach cancer. The review indicates the emergence of potentially effective and promising areas for the diagnosis and treatment of Hp-infection, including genetic modification of the Hp genome to ensure its existence as a human symbiont and a significant reduction in its pathogenic properties. At the same time, sequencing of a new generation reveals mutations in Hp genes associated with its pathogenicity and antibiotic resistance, which contributes to the creation of new classes of drugs to increase the effectiveness of Hp eradication with a decrease in the proportion of antibiotics and an increase in pathogenetically significant inhibitors of its pathogenic factors for the prevention of side effects, a significant reduction in the empirical use of several antibiotics and increased compliance to treatment.

Background: Estimating the excess of premature deaths (before the age of 75 years) and Potential Years of Life Lost allows ranking causes of death as an expression of the burden of disease in a population. We statistically analysed the impact of the coronavirus disease 2019 (COVID-19) pandemic on excess premature mortality in the total population and specifically, by sexes, compared to the pre-pandemic period, through Potential Years of Life Lost. Material and method: In this retrospective descriptive observational study, we counted excess of premature mortality in the years 2020, 2021, and 2022 by cause of death (cardiovascular diseases, cancer, digestive diseases, injury, COVID-19, and other causes) and by sexes compared to the period average from 2017-2019, based on the deaths registered in Bihor County (48,948 people). Results: Premature deaths due to COVID-19 (1,745 people of both sexes) contributed 71.3% to excess mortality, the population being similar for both sexes (71.4% in men and 71.2% in women). The Potential Years of Life Lost/death due to COVID-19 was 11.84 years for both sexes (11.76 years in men and 12.02 years in women). Potential Years of Life Lost/all-cause heath was lower during the pandemic (13.42 years for both sexes, 14.06 years for men and 12.32 years for women) compared to the pre-pandemic period (14.6 years for both sexes, 15.1 years for men and 13.5 years for women). Conclusions: The excess of premature mortality and decreased Potential Years of Life Lost/death during the pandemic, shows an increase in the proportion of deaths at ages closer to the established limit for premature mortality (75 years) compared to the pre-pandemic period.

Background: Chronic atrophic gastritis (CAG) is a chronic inflammatory disease and premalignant lesion of gastric cancer. As an antimicrobial peptide, hepcidin can maintain iron metabolic balance and is susceptible to inflammation. Objectives: The objective of this study was to clarify whether hepcidin is involved in abnormal iron metabolism and ferroptosis during CAG pathogenesis. Methods: Non-atrophic gastritis (NAG) and chronic atrophic gastritis (CAG) patient pathology slides were collected, and related protein expression was detected by immunohistochemical staining. The CAG rat model was established using MNNG combined with an irregular diet. Results: CAG patients and rats exhibited iron deposition in gastric tissue. CAG-induced ferroptosis in the stomach was characterized by decreased GPX4 and FTH levels and increased 4-HNE levels. Hepcidin, which is mainly located in parietal cells, was elevated in CAG gastric tissue. The high gastric level of hepcidin inhibited iron absorption in the duodenum by decreasing the protein expression of DMT1 and FPN1. In addition, the IL-6/STAT3 signaling pathway induced hepcidin production in gastric tissue. Conclusion: Our results showed that the high level of gastric hepcidin induced ferroptosis in the stomach but also inhibited iron absorption in the intestines. Inhibiting hepcidin might be a new strategy for the prevention of CAG in the future.
