ObjectivesWe aimed to develop and validate a nomogram integrating clinical and sonographic characteristics for the individualized SUI risk evaluation in the early postpartum stage.MethodsThis was a prospective cross‐sectional study. From June 2020 to September 2022, singleton primiparas who underwent TPUS examination at 6–8 weeks postpartum were recruited. They were divided into the training and validation cohorts at a ratio of 8:2 according to the temporal split. All subjects were interviewed before TPUS examination. Univariate and multivariate logistic analyses were performed to develop three models: the clinical, sonographic, and combined models. The ROC curve was plotted to evaluate model discrimination ability. Finally, the combined model was selected to establish the nomogram. The nomogram's discrimination, calibration, and clinical usefulness were evaluated in the training and validation cohorts.ResultsThe performance of the combined model was better than that of the clinical and sonographic models. Six predictors (BMI, delivery mode, lateral episiotomy, SUI during pregnancy, cystocele, and bladder neck funneling) remained in the combined model. The nomogram based on the combined model had good discrimination with AUCs of 0.848 (95% CI: 0.796–0.900) and 0.872 (95% CI: 0.789–0.955) in the training and validation cohorts, respectively, and the calibration curve showed good efficiency in assessing postpartum SUI. Decision curve analysis showed that the nomogram was clinically useful.ConclusionsThe nomogram based on clinical and sonographic characteristics showed good efficiency in assessing postpartum SUI risk and can be a convenient and reliable tool for individual SUI risk assessment.

Introduction and hypothesis This study aimed to establish a risk prediction model for postpartum stress urinary incontinence (SUI) based on pelvic floor ultrasound measurement data and certain clinical data. Methods Singleton pregnant women aged ≥ 18 years who underwent delivery were selected. All participants were followed up to determine the symptoms of SUI, and pregnancy-related data were collected at the time of registration. Pelvic floor ultrasound was performed at 6–12 weeks postpartum to obtain ultrasonic measurement data. Logistic regression analysis was used to select predictors and establish a nomogram to predict the risk of postpartum SUI. Area under the ROC curve (AUC) values and calibration curves were used for discrimination and calibration, respectively. Finally, external verification of the model was carried out. Results A total of 255 participants were included in the analysis, comprising 105 in the postpartum SUI group and 150 in the non-SUI group. Logistic regression analysis identified age, parity, vaginal delivery, bladder neck descent (BND), and angle of internal urethral orifice funnel as risk factors for postpartum SUI (all P < 0.05). Conclusions We constructed a prediction model for postpartum SUI based on pelvic floor ultrasound measurement data and certain clinical data. In clinical practice, this convenient and reliable tool can provide a basis for formulation of treatment strategies for patients with postpartum SUI.

Objective to explore the diagnostic value of perineal four-dimensional ultrasound (4D-US) in postpartum pelvic floor dysfunction (PFD) disease. 328 postpartum PFD patients diagnosed by clinical pelvic floor palpation from June 2018 to December 2020 were selected as the PFD group, and 328 patients without PFD were selected as the control group. All participants underwent perineal 4D-US, the indicators were statistically analyzed. The results showed that the LAT of left and right, LHLR, LHAP, LHA, resting state and holding the breath in the PFD group were higher than those in the control group, and the difference was statistically significant (P<0.05). From cervix to lower margin of pubic symphysis, bladder to lower margin of pubic symphysis of the pubic symphysis, and from the ampulla of the rectum to the lower margin of the pubic symphysis, the PFD group was larger than the control group, but the result of urethral rotation was reversed, and the difference was statistically significant (P<0.05). The morphologic features of the levator ani muscle and pelvic fissure can be detected early using 4D-US, which is a reliable technique that can be learned in a short period of time.
