Background: Hepatitis C is a liver disease caused by hepatitis c virus (HCV is RNA virus). The virus can cause both acute and chronic infection, ranging in severity from a mild illness lasting a few weeks to a serious, lifelong illness 1 . The hepatitis c virus is blood born virus and is most common modes of infections are through unsafe injection practices; inadequate sterilization of medical equipments; and transfusion of unscreened blood and blood products. 2 Objective: To evaluate Doppler Sonographic changes of portal vein in chronic hepatitis C versus normal healthy population.Methodology: Different types of Doppler ultrasound machines Toshiba, Aloka and Esote equipped with 3.5-5 MHZ curve linear multi-frequency transducer was used to scan the liver and portal venous system.Results: In this study, 40 patients were scanned, 20 of them were HCV positive and 20 were normal subjects. out of 20 HCV positive patients, 13 were male and 7 were female, while in normal subjects 11 were male and 9 were female The mean portal vein flow velocity in HCV positive patient was 11.1015 ± 2.6470 which is reduced as compared to normal subjects 20.4400 ±4.37558. The portal vein flow direction in normal subjects was hepatopetal flow while in HCV positive patients flow was hepatofugal flow in 12 patients and hepatopetal in 8 patients.Conclusion: We conclude from our results that HCV positive patient population shows various changes in liver size, echo texture, blood flow characteristics and some other associated features like ascites and collateral formation as a result of portal hypertension.

Background:Doppler ultrasonography (Doppler US) plays an important role in evaluating patients with liver cirrhosis. This study aims to investigate the hemodynamic alterations of hepatic artery and portal vein among children with liver cirrhosis and portal hypertension (esophageal varices).Materials and Methods:We conducted an analytical cross-sectional study in Imam Hossein Children's Hospital, Isfahan, Iran, in 2016. A number of 33 cirrhotic children with or without esophageal varices were selected through convenience sampling method to be compared with 19 healthy children as controls using color and spectral Doppler US.Results:Portal vein mean velocities were 15.03 ± 7.3 cm/s in cirrhotics, 16.47 ± 6.4 cm/s in controls (P = 0.51), 11.6 ± 4.7 cm/s in patients with varices, and 17.9 ± 7.3 cm/s in patients without varices (P = 0.015). Mean diameters of caudate lobe, portal vein, and splenic vein, as well as the mean values of liver and spleen span, were significantly higher in cirrhotic children. The frequency of flow reversal (hepatofugal flow) was not detected significantly different in cirrhotics. Peak systolic velocity, end diastolic velocity, pulsatility index, and resistive index for hepatic artery as well as liver vascular index were not significantly different in cirrhotics in comparison with controls.Conclusion:Alterations in Doppler parameters of portal vein including diameter and velocity may be the helpful indicators of liver cirrhosis and esophageal varices in children, respectively. Parameters of hepatic artery may not differentiate children with liver cirrhosis.

Introduction: Liver Cirrhosis is associated with significant morbidity and mortality. Simple parameters such as portal vein pulsatility index(PI) and complete spectral widening(CSW) have been observed in some studies to reflect early alterations in portal hemodynamics associated with the severity of liver disease. Methodology: It was a cross-sectional study, and a purposive non-probability sampling technique was used for selecting cases. Results: Among 33 patients with liver disease, 5 (15.2%) of them were in Child-Pugh class A, 10 (30.3%) in class B and 18 (54.5%) in class C. In the healthy group, 97.0% (32 of 33) had PI between 0.2–0.5 and 3.0% (1 of 33) had pronounced pulsatility > 0.5 whereas, in the cirrhotic patients, 63.6% (21 of 33) had PI between < 0.2 and 36.4% (12 of 33) had a pulsatility index between 0.2–0.5. The mean PI in Child-Pugh class A was 0.28 ± 0.11, in Child-Pugh class B 0.20 ± 0.03, and in Child-Pugh class C, it was 0.14 ± 0.05, indicating that the PI becomes lower with increasing severity of cirrhosis. Complete spectral widening (CSW) was present in 60.6% of patients with cirrhosis (20 of 33) and none of the healthy subjects in the control group (0 of 33). Conclusion: A decrease in the pulsatility index and the presence of complete spectral widening are valuable predictors of cirrhotic patients with portal hypertension, with alterations become significantly more pronounced as disease severity increases. However, this study could emphasize the role of duplex Doppler in recognizing disease in its early stages and determining disease severity in low setting tertiary hospital.
