Correlation of antibodies against desmogleins 1 and 3 with indirect immunofluorescence and disease status in a Greek population with pemphigus vulgaris

Abstract: Serum testing of anti-Dsg1 and anti-Dsg3 in PV patients not only provides significant correlations with IIF, treatment and disease type, but may serve as a monitoring tool for clinical course and treatment guidance.

“…In contrast, other studies did not find a correlation during follow-up [9,16,20]. In our study, we observed a better but not significant association for Dsg3 than Dsg1 ELISA, which was also observed by Herrero-González et al [17], in contrast with four other studies which found a better correlation for Dsg1 ELISA [10,18,21,22]. …”

“…Although our study included a limited number of patients, we did not find any significant association of Dsg ELISA values and clinical status. Only a few previous studies with a similar or higher number of patients (from 40 to 63) compared to our study published results regarding the interest of Dsg ELISA during follow-up [8,9,10,11]. Dsg ELISA has >90% sensitivity and specificity at diagnosis [4,5,12], but studies show paradoxical results during follow-up [8,9].…”

“…A correlation between ELISA scores and disease activity has been assessed in several studies and it was proposed that ELISA values tend to correlate with the severity of the disease (2,10,13,14,(16)(17)(18)(19)(20)(21)(22)(23)(24). More precisely, skin disease severity was initially suggested to be related to Dsg1 antibody levels, whereas Dsg3 antibody levels correlate directly with the involvement of mucous membranes (4,21).…”

“…Dette kan dermed skille tilstanden fra dermatitis herpetiformis, der man ser IgA-nedslag i dermale papillae (9), og fra pemphigus vulgaris, der man finner karakteristiske IgG-avleiringer i epidermis (10).
