Correlation Between Vasoactive-Inotropic Score and Postoperative Acute Kidney Injury after Cardiovascular Surgery

Abstract: Background: Acute kidney injury (AKI) is a common complication of cardiovascular surgery. The aim of this study was to investigate the correlation between Vasoactive-Inotropic Score (VIS) and postoperative acute kidney injury in adult patients with cardiovascular surgery. Methods: We retrospectively reviewed the data of 1935 adult patients who underwent cardiovascular surgery between September 2017 and May 2019. The data of patients included demographic data, laboratory findings, intraoperative details, and po… Show more

“…Based on the literature research, the following five of the nine significant variables were included into multivariate binary logistic regression (see Table 3 ): ECMO [ 9 , 10 , 11 ], all cause infection after surgery [ 12 , 13 ], levosimendan [ 14 , 15 , 16 ], duration of NE therapy [ 17 , 18 , 19 ] and increase in Tacrolimus plasma levels [ 18 , 20 , 21 ]. Evidence for the choice to include these five variables can be found as references next to each variable.…”

“…In the multivariate analysis, we observed that hematologic oncologic disease, thrombocytopenia, and MODS are independently associated with the development of AKI. As an indicator of hypotension, increased inotropic requirements and higher VIS scores were associated with the occurrence of AKI 8 17 . The maximum value of the VIS score was found to be an independent predictor of postoperative AKI in adults 17 .…”

“…Therefore, we decided to use the vasoactive-inotropic score (VIS) change pre-operatively to post-operatively as our primary endpoint. VIS has been validated in adult cardiac surgery and has been found to be an independent predictor of combined morbidity and mortality [ 25 , 26 ] with apparently promising prospects in future study designs [ 27 ]. Considering the effect of a decreased need for vasoactive medications concomitant with the CytoSorb ® -mediated attenuation of hyper-inflammation associated vasoplegia in our patients, we expected the VIS change from pre-op to post-op to be improved in the CytoSorb ® group compared to controls.…”
