Correlation between penile duplex findings and stress electrocardiography in men with erectile dysfunction

Abstract: The aim of this work is to assess the association between vasculogenic erectile dysfunction (ED) and coronary artery disease in men above the age of 40 y. The study included 40 patients above 40 y of age with vasculogenic ED of more than 3 months duration. A dynamic duplex study after intracavernosal injection of a bimix solution (60 mg papaverine þ 2 mg phentolamine mesylate) was carried out using a color ultrasound machine. The patients underwent a stress ECG test, carried out on a motor-driven treadmill acc… Show more

“…Previous epidemiological studies have demonstrated a close relationship between ED and cardiovascular disease and suggested the hypothesis that vascular ED may be an early marker of cardiovascular disease. 26 Montorsi et al 27 have reported that in 70% of men undergoing 29 Altogether, these findings suggest that the endothelial dysfunction, which is a marker of early stages of atherosclerosis, has an impact on erectile function both in organic and psychogenic patients 30 being the penile circulation mainly dependent upon correct endothelial function and less on metabolic-induced vasodilation. Indeed, in accordance with the data previously reported by Manciniet al, 31 our patients showed pathological baseline PSVs at Duplex ultrasound that were consistent with the presence of endothelial dysfunction in almost all of our patients; a significant amelioration after ADs tadalafil administration with parallel improvement of penile FMD after shear stress occurred.…”

“…8,10,11,13,37 Furthermore, men undergoing radiation were more likely to report a history of cardiovascular or peripheral vascular disease, both of which are associated with erectile dysfunction. 39 The preponderance of these comorbid conditions may have further biased our radiation cohorts toward worse sexual function.…”

“…Shamloul et al (30) reported penile peak systolic velocities less than 35 cm/s to have 100% specificity for predicting ischemic heart disease. In patients with angiographically proven CAD and clinical ED, a study reported that 67% experienced symptoms of ED before coronary symptoms, with sexual symptoms occurring a mean of 38.8 months earlier.…”
