Purpose To investigate the tear meniscus using Fourier-domain optical coherence tomography (FD-OCT) in patients with acne rosacea and to determine the relationship between tear film break-up tests (TBUT), Schirmer test and FD-OCT values and to compare them with that of healthy controls. Methods One hundred and four patients with acne rosacea and 104 body mass index age-sex-matched healthy subjects were enrolled in this prospective case-control study. Tear meniscus height (TMH), tear meniscus depth (TMD), and tear meniscus cross-sectional area (TMA) were measured using FD-OCT (RTVue-100); TBUT, Schirmer test and dry eye questionnaire (OSDI) were evaluated. Correlation was assessed using Spearman's correlation coefficient. Results Mean TMH, TMD, TMA, TBUT, and Schirmer test values of acne rosacea patients were significantly lower than and OSDI score significantly higher than those of controls. Tear mensicus values were significantly correlated with OSDI scores, TBUT, and Schirmer test results. The intraobserver intraclass correlation coefficient (ICC) for tear parameters ranged from 0.90 to 0.96 and interobserver ICC ranged from 0.92 to 0.94, which revealed high reproducibility for all measurements parameters both acne rosacea and control subjects. Accuracy of dry eye diagnosis by FD-OCT was 60.4% for TMH, 58.7% for TMD, and 64.8% for TMA. Conclusion Tear meniscus measurement with FD-OCT could provide acceptable specificity and sensitivity in diagnosis of dry eye in acne rosacea patients.

ÖzetAmaç: Spektral optik koherens tomografi (OKT; RTVue, Optovue) ile belirlenen alt gözyaşı menisküs (GM) parametrelerinin klinik testler ve hasta semptomları ile ilişkisini değerlendirmek ve GM parametrelerinin kuru göz tanısındaki duyarlılık ve özgüllüğünü incelemek. Ge reç ve Yön tem: Bu prospektif çalışma kapsamında 38 hastanın (22 kuru göz hastası, 16 sağlıklı birey) 38 gözü değerlendirildi. Tüm hastalarda rutin oftalmolojik muayeneyi takiben ön segment OKT ile belirlenen GM yüksekliği (GMY), GM derinliği (GMD) ve GM alanı (GMA) ölçüldü, gözyaşı kırılma zamanı (GKZ) ve anestezili Schirmer testi değerlendirildi. Hasta semptomlarının değerlendirilmesi için katılımcılar oküler yüzey hastalığı indeksi (OSDİ) anketini tamamladı. Testler arasındaki korelasyon Pearson bağıntı katsayısı (r) ile değerlendirildi. So nuç lar: Schirmer sonuçları ile GMY, GMA ve GMD arasında anlamlı pozitif korelasyon vardı (sırasıyla r=0,79, 0,58, 0,58). GKZ ile GMY arasında anlamlı pozitif korelasyon var iken GKZ ile GMA ve GMD parametreleri arasında anlamlı korelasyon yoktu (sırasıyla r=0,63, 0,14 ve 0,10). OSDİ skoru ve GM'nün 3 parametresi arasında korelasyon bulunmadı. GM parametreleri kuru göz grubunda kontrol grubuna göre daha düşük idi. Kuru göz tanısı koymada duyarlılık ve özgüllük sırasıyla GMY için %81,5 ve %86,8; GMA için %78,9 ve %76,3; GMD için ise %76,3 ve %52,6 idi. ABSTRACT.Purpose: To describe a new method of measuring early phase tear clearance by anterior segment optical coherence tomography (AS-OCT). Methods: Sixty normal subjects were divided into a young group (30 subjects; 29.6 AE 7.2 years) and an elder group (30 subjects; 71.4 AE 10.8 years). AS-OCT (CASIA SS-1000, Tomey, Japan) with customized software was used to record the tear meniscus at the centre of the lower eyelid. Five microlitres of lukewarm saline solution was dropped into the lower conjunctival sac, and an image of the tear meniscus was obtained immediately and again 30 seconds after natural blinking. The tear meniscus height (TMH) and tear meniscus area (TMA) were measured in the AS-OCT images, and the percentage decrease in the TMH and TMA was used as a measure of the tear clearance. Correlations between tear clearance and clinical features including degree of conjunctivochalasis, degree of protrusion of inferior lacrimal punctum, distance of lacrimal punctum from the Marx line and fluorescein clearance rates were also determined in another healthy population consisting of 30 subjects. Results: The OCT tear clearance rate was 35.2 AE 11% for TMH and 28.1 AE 12.4% for TMA in the young group, and 12.4 AE 7.3% and 6.2 AE 9.1%, respectively in the elder group. The differences were significant for both the TMH (p = 0.017) and the TMA (p = 0.024). The OCT-determined tear clearance was positively correlated with the fluorescein clearance rate, and negatively correlated with the distance between the lacrimal punctum and Marx line, degree of conjunctivochalasis and degree of lacrimal punctum protrusion. Conclusion: AS-OCT can be used as a rapid, non-invasive and quantitative method of determining the early phase tear clearance rate in a normal healthy population.
