Optical coherence tomography angiography (OCTA) has increasingly become a clinically useful technique in ophthalmic imaging. We evaluate the repeatability and reproducibility of blood perfusion in the optic nerve head (ONH) measured using optical microangiography (OMAG)-based OCTA. Ten eyes from 10 healthy volunteers are recruited and scanned three times with a 68-kHz Cirrus HD-OCT 5000-based OMAG prototype system (Carl Zeiss Meditec Inc., Dublin, California) centered at the ONH involving two separate visits within six weeks. Vascular images are generated with OMAG processing by detecting the differences in OCT signals between consecutive B-scans acquired at the same retina location. ONH perfusion is quantified as flux, vessel area density, and normalized flux within the ONH for the prelaminar, lamina cribrosa, and the full ONH. Coefficient of variation (CV) and intraclass correlation coefficient (ICC) are used to evaluate intravisit and intervisit repeatability, and interobserver reproducibility. ONH perfusion measurements show high repeatability [CV ≤ 3.7% (intravisit) and ≤ 5.2% (intervisit)] and interobserver reproducibility (ICC ≤ 0.966) in all three layers by three metrics. OCTA provides a noninvasive method to visualize and quantify ONH perfusion in human eyes with excellent repeatability and reproducibility, which may add additional insight into ONH perfusion in clinical practice. © The Authors. To assess vascularity of the macular area in patients with glaucoma using optical coherence tomography angiography (OCT-A) and evaluate the role of its examination in early glaucoma diagnosis. Materials and Methods: Thirty-eight eyes of patients with the early stage of primary open-angle glaucoma (POAG), 27 eyes of patients with the advanced and far-advanced stages of POAG, and 22 eyes of age-matched healthy subjects were examined using spectral-domain OCT-A (SD-OCT-А) (RtVue xR Avanti with the AngioVue software, San Jose, CA, USA) in order to measure retinal thickness and angio flow density (AFD) retina in macula (an area bounded by a circle with a diameter of 3 mm), inсluding fovea and parafovea regions (superficial and deep) of the inner retinal layers. The AFD disc and peripapillary flow density were measured in optic nerve head (ONH) and 750-μm-wide elliptical annulus extending from the optic disc boundary. Retrobulbar blood flow parameters, including ophthalmic artery (OA), central retinal artery (CRA), short posterior ciliary arteries (PCAs), central retinal vein (CRV), and vortex veins (VV), were measured by color doppler imaging (CDI). The average thickness of the ganglion cell complex (avg GCC), retinal nerve fiber layer (RNFL), and choroid, as well as the focal loss volume (FLV) and global loss volume (GLV) of GCC were measured by means of SD-OCT. Automated perimetry was performed using Humphrey perimeter (Carl Zeiss Meditec, Dublin, CA, USA). Corneal-compensated intraocular pressure (IOPcc) and corneal hysteresis (CH) were determined using ocular response analyzer. Mean ocular perfusion pressure (MOPP) was calculated by measuring IOP and arterial blood pressure (BP) immediately prior to OCT and using formula: MOPP = (2/3 diastolic BP + 1/3 systolic BP) × 2/3 -IOP. Statistical analysis was performed using SPSS version 21 and MASS library in the R language. As a measure of the parameter's importance in distinguishing patient groups, a value of the adjusted standardized statistic of the Mann-Whitney test (z-value) and an area under the receiver operating characteristic (ROC) curve (AUC) were used. Objectives This study aims to investigate real‐time elastography (RTE) use in the evaluation of the optic nerve head (ONH) and peripapillary structures for the diagnosis of primary open angle (POAG) and pseudoexfoliation (PEX) glaucoma. Methods This case–controlled study included 30 patients with POAG, 30 patients with PEX glaucoma, and 30 age‐matched control subjects. All of the participants underwent comprehensive ophthalmological examinations covering vessel density of optic nerve and retinal nerve fiber layer (RNFL) thickness measurements with optical cohorence tomography angiography and mean deviation (MD) measurements with Humphrey II Perimetry Visual Field Analyzer. In vivo evaluation of the biomechanical properties of the ONH and peripapillary structures were performed with RTE in all participants. Results We observed higher ratios of orbital fat to optic nerve head (ROFON) values (P = .008) and strain ratios of orbital fat to scleral–choroidal–retinal complex (ROFSCR) values (P = .004) in the POAG group compared with PEX glaucoma group and higher ROFON (P = .012) and ROFSCR values (P = .004) in PEX glaucoma group than the control group. ROFON and ROFSCR values were positively correlated with glaucoma duration and negatively correlated with MD, radial peripapillary vessel density (RPCVD), and inside disc vessel density in both glaucoma groups (P < .005; only in the PEX glaucoma group for MD and ROFSCR, P = .445). Conclusion Determining the biomechanical properties of ONH and peripapillary structures with RTE in glaucomatous eyes may offer a new perspective on the diagnosis and follow‐up of the progression of the disease.

Abstract: Angiograms demonstrated a reduced disc flow index and vessel density in glaucoma, and this reduction was closely related to GCC thickness. This indicated that measurement of disc perfusion by angio-OCT might be important for the monitoring of glaucoma.
