Background: The installation of drainage after MRM surgery has been aimed at reducing seroma formation. However, when the drain should be removed has not been studied well. This study aims to determine the association of early drain removal with seroma formation. Methods: A nested cohort study was conducted on 40 breast cancer patients undergoing MRM surgery. Patients were divided into 2 groups; off-drain day I and day III post-surgery. Seroma formation was assessed 7 days after the patient was discharged. Results: Seroma formation seven days after discharge occurs in 25 of the total 40 patients (62.5%). There was no difference in seroma incidence between groups of patients on the off-drain day I and day III seven days after discharge (65% vs. 60%) (p 1,000) or the number of seromas (157.31 +120.00 ml vs. 149.58 + 110.00 ml) (p 0.437). Conclusion: Removing the drain faster does not reduce the incidence and number of seromas seven days after discharge.

Background: Acellular dermal matrix (ADM) is commonly used in implant-based breast reconstruction due to improved soft-tissue support and control of the implant pocket and decreased capsular contracture. However, concerns about complications have prompted the FDA to request more clinical data. This large-scale study aims to examine perioperative outcomes of ADM use in breast reconstruction. Methods: This study utilized a national insurance-based database to identify patients who underwent mastectomy between 2011 and 2019, with and without ADM. The groups were matched for age, region, and comorbidities. Complications within 90 days were compared using univariate and multivariate analyses. Results: A total of 49,366 patients were identified with 26,266 patients in the ADM group and 23,100 in the non-ADM group. Infection rates (4.7% ADM versus 4.4% no ADM) and seroma rates (3.9% ADM versus 4% no ADM) were similar. However, the ADM group had a 1% higher rate of implant removal (4.9% ADM versus 3.9% no ADM, P < 0.001). In direct-to-implant procedures, ADM use was associated with higher explantation rates (8.2% versus 6.3%, P = 0.002). Multivariate analysis identified tobacco use, hypertension, depression, obesity, ADM usage, and direct-to-implant surgery as risk factors for implant removal. Conclusions: This study found comparable infection and seroma rates in implant-based breast reconstruction with and without ADM. ADM use was associated with a 1% higher risk of implant removal, with risk factors including tobacco use, obesity, hypertension, depression, and direct-to-implant procedures. Multicenter studies and registry data on prepectoral breast reconstruction are warranted to help interpret these findings.

Highlight: Modification of radical mastectomy with flap fixation treatment in seroma production in breast cancer patients was analyzed.The number of seromas from the drain removal time in patients undergoing modified radical mastectomy with flap fixation was reduced. Abstract: Seroma is the most common complication after mastectomy procedure. This study was conducted to analyse the treatment of flap fixation in seroma production after modified radical mastectomy in breast cancer patients. An interventional prospective clinical study in 35 female patients with breast cancer who underwent modified radical mastectomy with flap fixation in the period August-December 2018 and 35 patients without flap fixation through historical data of patient who underwent modified radical mastectomy in the period 2016-2017 at RSUD Dr. Soetomo Surabaya. Data of characteristic patients will be presented descriptively and analyzed statistically using the appropriate test. The result of the statistical test using Chi Square with a 95% significance level obtained a value of p = 0.000 (p <0.05), it was said statistically that there was a significant relationship between the treatment of flap fixation and the time of releasing drain in this study. The treatment of flap fixation will reduce drainage time by <10 days. The results of the odds ratio (OR) obtained OR = 16 and CI: 95% (4.094 – 62.528) which means that the treatment of flap fixation will reduce the releasing time of the drain 16 times more effectively than not performing flap fixation. Correlation between the treatment of flap fixation and the time of releasing drain was considered to be moderately positive (Contingency Coefficient Chi Square = 0.476). There was a significant reduction in the number of seroma assessed from the time of releasing drain faster in patients who underwent modified radical mastectomy with flap fixation.
