Correlation between macroscopic morphologic features and malignant potential of colorectal sessile adenomas

Abstract: The large creeping-type adenomas (> 3 cm) have a definitely high frequency of submucosal invasive carcinoma. Endoscopic polypectomy or local resection may be adequate for most sessile adenomas. However, we recommend segmental resection of the colon with regional lymph node dissection for a creeping-type adenoma that is larger than 3 cm because of increased risk of submucosal carcinomatous invasion and lymph node involvement.

“…3 Some investigators now consider these lesions to be a particular form of superficial-type tumour 1,2,4 or a specific type of sessile adenoma. 3,5 Although large, nonpolypoid, colonic neoplasms are thought to exist, their precise biological characteristics remain unknown. We previously investigated the extent of apoptotic cell loss in large, H Yamada, H Hasegawa, H Iino et al Large, non-polypoid colorectal adenomas non-polypoid adenomas and found dominant apoptosis in lower parts of the tumour, which is thought to be a characteristic related to the unique mode of development of such tumours.…”

“…We studied and attempted to elucidate statistically significant relationships between cancer location, MM-MP distance, and lymph node metastasis in patients with early colorectal cancers. Ishikawa et al 18 reported that macroscopic morphologic features relate to the malignant potential of colorectal sessile adenomas. Depressed-type and flattype lesions tend to invade the submucosa rapidly.…”
