Correlation between lumbar dysfunction and fat infiltration in lumbar multifidus muscles in patients with low back pain

Abstract: Background: Lumbar multifidus muscles (LMM) are important for spinal motion and stability. Low back pain (LBP) is often associated with fat infiltration in LMM. An increasing fat infiltration of LMM may lead to lumbar dysfunction. The purpose of this study was to investigate whether there is a correlation between the severity of lumbar dysfunction and the severity of fat infiltration of LMM. Methods: In a cross-sectional study, 42 patients with acute or chronic LBP were recruited. Their MRI findings were visua… Show more

“…Most studies used the center of the intervertebral disc 10,21,[27][28][29][30]39,43,45,50,52,53,56,[59][60][61][62]65,71,74,79 or the superior/inferior endplate 9,15,19,20,26,31,34,35,41,44,[47][48][49]57,63,64,66,[68][69][70]82,83,85 as the level of slice. These slices are common and available in clinical practice.…”

“…Visual Semiquantitative Parameters Our review showed that several early studies used semiquantitative visual grading with distinct cut-off points (2-point scale 19,67 , 19,67 , 3-point scale 9,16,49,64,68,78 , 4-point scale 7,15,18,29,34,37,48,50,59,80 , and 5-point scale 17,28,35,51 ). When muscles were graded, the interobserver and intraobserver agreements were both acceptable with or without cut-off points 7,34,51 .…”

“…Apart from enhanced efficiency, automated methods also facilitate observer-independent processing. 8 The fat-fraction of muscle tissue is not only relevant for the assessment of neuromuscular diseases, but is also utilized for investigations of other diseases such as lower back pain 9,10 as well as traumatic neck pain. 11,12 Approaches for segmentation so far make use of unsupervised techniques, such as active contours, 13 clustering, 14,15 random walks, 16,17 or atlas approaches.…”

“…Fat infiltration of the multifidus muscle on MRI was graded as "grade 0" for 0-10% fat, "grade 1" for 10-50% fat, and "grade 2" for > 50% fat (Fig. 1) [9]. Apart from these studies, there are studies evaluating muscle cross-sectional areas [10,11].…”
