BackgroundFrailty refers to the loss of physiologic complexity and the associated decline in ability to withstand stressors as one gets older. It is defined as unintentional weight loss, exhaustion, weakness, slow walking speed, and low physical activity. According to several western studies, frailty is associated with cognitive impairment, but there have been few studies about the relationship between frailty and cognitive impairment in Korea. Thus, the purpose of this study is to analyze the relationship between cognition and factors related to frailty such as grip strength, walking speed, physical activity, and depression, among female patients older than 65 in Korea.MethodsA total of 121 subjects among the outpatients of the department of family medicine at Kangbuk Samsung Hospital who did not meet the exclusion criteria were included in this study. We divided the participants into 2 groups, according to the Korean version of the Montreal Cognitive Assessment (MoCA) score: 1 group with subjects that had normal cognition and the other group with patients that had impaired cognition. A comparison was made between the 2 groups in regards to the factors related to frailty, and we completed equation that predicting cognition from the frailty related factors.ResultsCompared with the impaired cognition group, the subjects in the normal cognition group had higher hand strength, and walked faster (P<0.001). There was no statistically significant difference in physical activity between the 2 groups (P=0.19). When multiple linear regression analysis was performed using age, grip strength, and walking speed as the predictor variables and MoCA score as the dependent variable, the regression coefficients were calculated to be: -0.2015, 0.2294, 1.2372, and -0.1436, respectively (P<0.05).ConclusionIn Korean female patients who are older than 65 years of age, cognition tends to decline as grip strength decreases, walking speed gets slower, depression becomes more severe, and as age increases.

Background Worldwide aging of the population leads to an increased number of older, more complicated patients attending emergency departments (ED). Presenting symptoms of various conditions can be vague in this population, leading to delayed and potentially missed diagnoses. In the current study, we sought to characterize the demographics, final diagnoses, and outcomes of older patients who are referred to the ED for generalized weakness. METHODS We conducted a retrospective observational study in an urban tertiary hospital ED. Participants were patients aged 65 years or older who were referred to a geriatric evaluation in the ED. A total of 2,226 patients met study eligibility criteria and were enrolled between April 1, 2010 and March 31, 2013. Data on physical characteristics, discharge diagnoses, and vital status were obtained from an electronic medical database and the Ministry of the Interior. RESULTS There was no significant difference between the “weakness” and “non-weakness” groups for patients who were discharged from the ED. For admitted patients, statistically significant differences (although of no clinical significance) were noted for white blood cells (WBC) and sodium levels. Both statistically and clinically significant differences were observed for functional level. The main discharge diagnosis for admitted patients was an infection (24%), followed by "nonspecific diagnoses" (22%). Social issues as the main diagnosis were attributed to < 1% of the patients. Patients who were hospitalized for weakness were less likely to survive during the following year compared with patients hospitalized for non-weakness, but that was not the case for patients who were not admitted. CONCLUSIONS Patients admitted due to weakness were significantly more dependent on others for carrying out their daily activities. They were also more likely to die during and following hospitalization. Most of them were diagnosed during the hospitalization as having a specific medical condition.

Background: Frailty, a multidimensional geriatric syndrome, confers a high risk of falls, disability, hospitalization, and mortality. The prevalence and correlates of frailty in older people dwelling in rural communities are unknown. Methods: We examined frailty, as defined by the Korean Geriatric Society, and social correlates among rural communitydwelling older adults (n=419) aged 65 or older. Results: The prevalence of frailty and prefrailty was 11.5% and 38.7%, respectively. Among eight frailty criteria, the prevalence of multiple medications was highest (57.3%), while self-rated health as "unhealthy" was the second highest (45.1%). In contrast, the prevalence of exhaustion was the lowest (10%) among the eight frailty criteria. The prevalence of frailty was 12.6% among women and 6.3% among men (p=0.02). Uneducated older adults showed a significantly higher prevalence of frailty (13.1%) than any other educational groups (6.3-9.2%). In addition, older adults with high social support showed lower frailty than the other groups. After adjustment for other factors; however, a lack of social support (odds ratio for frailty, 1.98; 95% confidence interval, 1.14-3.44) was associated with increased odds of frailty. Conclusion: Frailty is not uncommon among rural community-dwelling old men and women in Korea. In the rural community, females with a lack of social support might be at a higher risk of frailty, as those were two of the strongest risk factors discovered in this study. This study calls for more interest and concern for the frail older adults with a lack of social support in rural communities.
