Correlates of Illness Severity in Infectious Mononucleosis

Abstract: Infectious mononucleosis is caused by Epstein-Barr virus and can lead to complications, including hepatitis and hematological abnormalities, in a subset of patients. The authors of this article assessed measures of illness severity as well as viral load at presentation and six weeks later among a cohort of individuals <25 years of age for the purpose of informing the management of patients with infectious mononucleosis.

“…In one study, however, the specifics of the severity measurements were not detailed [9], and in the other, the scale used was not validated for this use [10]. We are aware of two other tools that have been used to measure the severity of acute IM, but these scales also have not been validated either in general or for IM specifically [11,12]. As part of a study to assess risk factors for the development of CFS following IM, we developed and then attempted to validate a scale for rating the severity of IM, by first reviewing the literature available prior to beginning our study for risk factors for the severity of acute IM [13][14][15][16][17][18].…”

“…Disease severity and duration correlate much better with e.g. CD8+ cell counts than with the viral kinetics itself and the expansion of the CD8+ cell count is controlled in asymptomatic EBV infection despite virus loads similar to those experienced in symptomatic EBV infection [1,2,1014]. Hence current understanding suggests that IM is caused by overreaction by the immune system, rather than EBV infection per se (viremia or B-cell expansion).…”
