ABSTRACT. Background. Infant and under-5 childhood mortality rates in developing countries have declined significantly in the past 2 to 3 decades. However, 2 critical indicators, maternal and newborn mortality, have hardly changed. World leaders at the United Nations Millennium Summit in September 2000 agreed on a critical goal to reduce deaths of children <5 years by two thirds, but this may be unattainable without halving newborn deaths, which now comprise 40% of all under-5 deaths. Greater emphasis on wide-scale implementation of proven, cost-effective measures is required to save women's and newborns' lives. Approximately 99% of neonatal deaths take place in developing countries, mostly in homes and communities. A comprehensive review of the evidence base for impact of interventions on neonatal health and survival in developingcountry communities has not been reported.Objective. This review of community-based antenatal, intrapartum, and postnatal intervention trials in developing countries aimed to identify (1) key behaviors and interventions for which the weight of evidence is sufficient to recommend their inclusion in communitybased neonatal care programs and (2) key gaps in knowledge and priority areas for future research and program learning.Methods. Available published and unpublished data on the impact of community-based strategies and interventions on perinatal and neonatal health status outcomes were reviewed. Evidence was summarized systematically and categorized into 4 levels of evidence based on study size, location, design, and reported impact, particularly on perinatal or neonatal mortality. The evidence was placed in the context of biological plausibility of the intervention; evidence from relevant developed-country studies; health care program experience in implementation; and recommendations from the World Health Organization and other leading agencies.Results. A paucity of community-based data was found from developing-country studies on health status impact for many interventions currently being considered for inclusion in neonatal health programs. However, review of the evidence and consideration of the broader context of knowledge, experience, and recommendations regarding these interventions enabled us to categorize them according to the strength of the evidence base and confidence regarding their inclusion now in programs. This article identifies a package of priority interventions to include in programs and formulates research priorities for advancing the state of the art in neonatal health care.Conclusions. This review emphasizes some new findings while recommending an integrated approach to safe motherhood and newborn health. The results of this study provide a foundation for policies and programs related to maternal and newborn health and emphasizes the importance of health systems research and evaluation of interventions. Objective: To describe a series of hospitalized patients with eczema craquelé (EC) and the possible correlations between clinical features of EC and cancer in an open prospective observational study. Patients and Interventions: The study population comprised 68 consecutive patients included between January 1, 1999 and December 31, 2000 who were followed up through December 2004. All patients who had localized or generalized EC were included. Patients underwent complete clinical examinations, routine laboratory tests, chest x-rays, abdominal ultrasound, and cutaneous biopsies performed on EC. Main Outcome Measures and Results: Rates of EC associated with cancer, clinical features of eczema, rate of recalcitrant eczema, relationship to other clinical prognostic factors, and paraneoplastic evolution were evaluated. Cancer was diagnosed in 32 patients (47%). We observed a significant difference in the presenting clinical signs of EC between patients with malignant tumors and patients without cancer. In patients with malignancies, EC was widespread on the trunk and we noted deep red and inflammatory fissures. In all cases, EC led to the discovery of malignancy or recurrence of cancer. Conclusion: Widespread EC, topical corticosteroid resistance, and deep red and inflammatory fissures were significantly correlated with neoplasia.

Wstęp Sucha skóra problemem dermatologicznymSuchą skórę spotyka się zarówno u zdrowych ludzi, jak i u pacjentów z chorobami skóry. Występuje w powiązaniu ze starzeniem się, jako odpowiedź na działanie czynników śro-dowiska i w kilku genetycznie uwarunkowanych schorzeniach (atopowe zapalenie skóry, łuszczyca, rybia łuska, keratodermia dłoni i podeszew). Charakteryzuje się odczuciem zgrubienia, szorstkości, łuszczeniem i widocznym popękaniem. Przyczyną jest spadek funkcji bariery naskórkowej, charakteryzujący się wzrostem ucieczki przeznaskórkowej wody [1].Suchość skóry jest ściśle powiązana z funkcją bariery naskórkowej, czyli bariery między nieprzyjaznym środowi-skiem zewnętrznym a organizmem. Znacząca rola przypada tutaj lipidom i keratynocytom warstwy rogowej naskór-ka. Nawodnienie i przeznaskórkowa utrata wody (transepidermal water loss -TEWL) stanowią fizjologiczne właściwości warstwy rogowej naskórka i są ze sobą ściśle powiązane. W zdrowej skórze stosunek pomiędzy TEWL a nawodnieniem jest wprost proporcjonalny. Jeżeli nastąpi uszkodzenie skóry lub spadek wydolności bariery naskór-kowej, nawodnienie warstwy rogowej się zmniejsza, a przeznaskórkowa ucieczka wody się zwiększa. Funkcja bariery jest również zależna od biologicznej aktywności żywych keratynocytów wytwarzających strukturalne białka i tłusz-cze, mających zdolności wytwarzania cytokin, które sterują zarówno procesami zapalnymi, jak i rozrostem naskórka. Keratynocyty podlegają różnicowaniu się podczas przechodzenia z warstwy podstawnej naskórka (gdzie zachodzi podział komórek), aż do zajęcia miejsca w warstwie rogowej jako dobrze wpasowane elementy struktury zwane blaszkami rogowymi. Woda w warstwie rogowej jest zwią-zana z keratyną blaszek rogowych oraz hydrofilowymi czę-ściami lipidów międzykomórkowych znajdujących się w dolnej części naskórka, między warstwą ziarnistą i rogową. Aby zapobiec całkowitej przeznaskórkowej ucieczce wody, lipidy polarne hydrofilowe ulegają przemianie w niepolarne formy lipidów hydrofobowych, które znajdują się w warstwie rogowej. W suchej skórze powstają zaburzenia w składo-wych białek i w układzie lipidów, czego wynikiem jest prawdopodobnie widoczne klinicznie łuszczenie [15].Za prawidłowe funkcjonowanie naskórka jest również odpowiedzialna zawartość wody w warstwie rogowej. Minimalna jej ilość nie powinna być mniejsza niż 10%. Woda do warstwy rogowej jest dostarczana z głębszych warstw naskórka przez gruczoły potowe ekrynowe, a także z otoczenia zewnętrznego. Na stan nawodnienia naskórka wpły-wają przede wszystkim rozpuszczalne w wodzie składniki tzw. naturalnego czynnika nawilżającego (natural moisturizing factor -NMF). Te higroskopijne substancje o małej masie cząsteczkowej to głównie aminokwasy, kwas pirolidyno-węglowy, mleczany i mocznik. Przypuszcza się, że substancje te są chronione przez lipidy błony komórkowej, któ-re pozwalają na przechodzenie wody z wewnątrz na zewnątrz. Uważa się, że elastyczność naskórka jest zależ-na nie tyle od ilości samej wody w warstwie rogowej, ile od obecności w niej składników NMF...
