A 25 years old female with limp and a gradually progressive flexion deformity at the knee, being able to walk only on crutches, following septic arthritis of the knee joint during childhood had a fixed flexion deformity of 70 degrees at the knee with a limb shortening of 7cm with evidence of multiple healed previous sinuses and secondary deformities of spine, pelvis and ankle. The soft tissue over the posterior aspect of knee joint was scarred with a degenerated and ankylosed knee joint. A supracondylar anteriorly based closing wedge osteotomy of the femur at the CORA was done to correct the deformity gradually using a long spanning Ilizarov's ring external fixator with corticotomy and sequential lengthening done later. At final follow up at 12 months after completion of treatment apart from pin tract infections the patient had no other complications like residual or recurrence of deformity, delayed consolidation, nonunion, refracture, peroneal nerve injury or hardware failure. Also with satisfactory hip and ankle range of motion and a limb that was pain free and in functional position unaided ambulation was possible. Hence, wedge resection and gradual correction with Ilizarov's technique can achieve desired results without causing any neurovascular complications or soft tissue procedure requirement in a chronic severely flexed ankylosed knee.

Introduction Soft-tissue origin joint contractures are a common orthopedic problem. It could be due to various etiologies. Treatment options are available from conservative to surgical methods. These joint contractures slowly become irreversible causing impairment in activities of daily routine. The Ilizarov method is a well established and time-tested method used for management of bone pathologies, but its use in the management of soft-tissue origin contractures is also possible. It has an established role in neoosteogenesis and histogenesis. Fixator assisted soft-tissue stretching done at sustained slow pace leads to histoneogenesis that avoids stretching of neurovascular structures and reduces the possibility of recurrence.Aims To determine usefulness of the Ilizarov method in management of joint contractures of soft tissue origin; to meet functional requirements of patients; to study complications of Ilizarov method in management joint contractures due to soft tissue origin.Material and methods A total of 6 cases of soft-tissue origin joint contractures due to tuberculosis, post‑traumatic stiffness, post-burn contracture, deformity due to a snake bite in the age group from 3 to 55 years were treated with gradual distraction of joint with the Ilizarov method from January 22 to October 23. Two cases were of triple knee deformity, two were post-traumatic elbow stiffness, one was post-burn great toe contracture and one was post snake bite valgus foot contracture. All cases were operated with transarticular Ilizarov frame application and gradual distraction of joints and soft tissue with the help of hinge- and rod distractor assembly done. All cases completed follow up of 1 year. Aggressive physiotherapy was given postoperatively.Results All cases obtained a reasonable functional outcome, with no recurrence of deformity. All patients walk independently.Conclusion The Ilizarov method can be used for treating joint contractures due to traumatic and non‑traumatic pathologies.

Background: limb deformity, especially lower leg deformity is a common case in orthopaedic practice. Limb deformity can be classified according to cause (Congenital, developmental, post trauma), geometry (angulation, rotation, shortening), severity and progression. In severe and complex deformity, the treatment choice is gradual correction using Ilizarov system. Benefit of the gradual correction using Ilizarov system are: (1) reduce the risk of neurovascular injury, (2) minimal soft tissue disturbance, (3) multiplanar and multidirectional correction.Cases: We have performed 11 deformity correction using ilizarov device during january 2013- September 2017. Evaluation data was obtained retrospectively from medical record. Nine patients were managed by Ilizarov device for bone lengthening indication and 2 patients for bone transport indication. The treatment result was evaluated using ASAMI scoring system for bone condition and functional condition.Results: Elevent patients were managed by Ilizarov system for gradual deformity correction. Nine patients already finished the treatment and the frame was removed. We cannot evaluate two patients because the correction was not finished. The ilizarov device was changed with other fixation because implant failure during correction phase. Evaluation using ASAMI score showed good bone score result for 9 patients and 7 excellent results for functional score.Conclusion: Ilizarov system has a promising result to correct lower limb deformity patients in Dr Soetomo General Hospital.
