Coronary calcifications and plaque characteristics in patients with end-stage renal disease

Abstract: ESRD is associated with diffuse atherosclerosis and calcific plaque morphology. Nonetheless, the association between the calcium score and atherosclerotic burden is not affected by the presence of ESRD.

“…31 A few studies assessed the diagnostic accuracy of cardiac CT in comparison to invasive coronary angiography in patients with ESRD. [32][33][34][35][36][37] Budoff and co-workers showed that coronary calcium score has a reasonable discriminative capacity for coronary atherosclerosis in ESRD patients, with areas under the curve of 0.77 and 0.75 for detecting coronary stenoses C50% and C70%, respectively. 32 Rosario et al showed that Agatston's score C400 was associated with a higher number of significant stenoses by invasive coronary angiography.…”

“…Coronary artery calcification (CAC) on CT is associated with coronary plaque accumulation (Jug et al 2013). Still, there are no reports comparing VC on plain radio-graphs to the degree of CAC or stenosis detected by CAG in patients undergoing dialysis.…”

“…Patients with ESRD hence have a high burden of CAC compared to a non-CKD population and the association with ischemia is very weak as demonstrated before. 4,8 However, few studies have demonstrated a correlation of CACS with luminal stenosis and longterm outcomes even in the ESRD population. [8][9][10] Although EAT has been associated with atherosclerotic burden and cardiac outcomes, it provided very little (if any) incremental information for predicting abnormal perfusion in this study.…”
