Coronary Artery Calcium Data and Reporting System (CAC-DRS): A Primer

Abstract: The Coronary Artery Calcium Data and Reporting System (CAC-DRS) is a standardized reporting method for calcium scoring on computed tomography. CAC-DRS is applied on a per-patient basis and represents the total calcium score with the number of vessels involved. There are 4 risk categories ranging from CAC-DRS 0 to CAC-DRS 3. CAC-DRS also provides risk prediction and treatment recommendations for each category. The main strengths of CAC-DRS include a detailed and meaningful representation of CAC, improved commun… Show more

“…Although the Agatston score is the most widely adopted method, it is not the only score to evaluate CAC. The calcium volume and calcium mass represent the total volume of calcified voxels and the “true mass” of calcium in the coronary tree, respectively [ 19 , 25 ]. Both calcium volume and calcium mass had higher interscan reproducibility compared to the Agatston score [ 19 , 26 , 27 ].…”

“…Absolute CAC scores are then assigned to each coronary vessel based on the maximal Hounsfield units (HU) and may be associated with a visual score such that 0 = none, 1–10 = minimal, 11–100 = mild, 101–400 = moderate, and >400 = severe. Visual scores are subjective estimations of CAC based on visual analysis of non-ECG-gated scans [ 6 ].…”

“…The majority of such deaths take place in low-and middle-income countries, accounting for nearly 7 million deaths and 129 million DALYs annually. Moreover, patients who survive myocardial infarction have at least a five-to six-fold higher annual mortality rate, compared to those who are not afflicted with CAD [2][3][4].…”
