Coronally repositioned periodontal flap.

Abstract: After increasing the width of the attached gingiva by free palatal mucosa transplants, 20 procedures with coronal flap repositioning were performed on 41 teeth with gingival recessions in 13 young adults. The amount of gingival recession and the clinical gingival sulcus depth were measured pre-operatively and 1, 6 and 12 months after surgery; the amount of osseous dehiscence was measured during surgery. No significant differences were found among reduction values of gingival recession by reattachment 1, 6 and … Show more

“…Miller's criteria for successful root coverage include: the soft tissue margin must be at the cemento-enamel junction, clinical attachment to the root, with sulcus depth of 2mm, and no bleeding on probing. [4] Thick (1.5 -2+ mm) free epithelialized autogenous soft tissue grafts are recommended for recession coverage. [9] Although these have a greater primary contraction as compared to the thin (0.5-0.8 mm) or average (0.9-1.4 mm) thickness grafts, their secondary contraction is less during the healing process.…”

“…[2] Later, Pennel et al in 1969 used the masticatory mucosa as the donor tissue and the technique is still in use. [3] FGG, used both as a two-step procedure (Bernimoulin et al 1975) -or a one step procedure (P.D. Miller 1982), for the coverage of denuded roots, is a versatile modality of treatment and can be used in a variety of clinical situations.…”

“…The coronally advanced flap was first described by Bernimoulin et al 20 The procedure can be performed either as a one stage technique to cover shallow recession defects 21 or a two stage technique which is combined with a free gingival graft, connective tissue graft or with guided tissue regeneration procedures. If the gingival tissue apical to the recession defect has thin gingival biotype or there is insufficient keratinised tissue, a free gingival graft or a connective tissue graft can be carried out first to increase the thickness and amount of keratinised tissue.…”

“…Periodontists then began to use free gingival grafts for esthetic corrections such as soft tissue recession coverage 3 , soft tissue augmentation of the ridges, and alveolar preservation in esthetic areas. Moreover, the evolution of the procedures for increasing soft tissues has been marked by a perpetual increase in the esthetic demands of our patients.…”
