Cord blood banks collect units with different HLA alleles and haplotypes to volunteer donor banks: a comparative report from Swiss Blood stem cells

Abstract: Allogeneic haematopoietic SCT is a standard therapy for many patients with haematological diseases. A major aim of public umbilical cord blood (UCB) banking is to establish an inventory with a large HLA diversity. Few studies have compared HLA diversity between UCB banks and volunteer unrelated donor (VUD) registries and examined whether UCB banks indeed collect more units with rare alleles and haplotypes. This study compares HLA-A/B/DRB1 allele frequencies and inferred A/B/DRB1-haplotypes in 1602 UCB units an… Show more

“…The HLA-A-B-DRB1 haplotype frequencies found in this study are very close (although with regional variations) to previous estimations done in 2008 by the ZKRD (the German donors registry) [7] and in 2010 by the BMDW on the whole Swiss data pooled together [45], and this is also true for haplotypes including the HLA-C locus. However, our approach has the advantage of taking ambiguities into account to estimate frequencies and is thus much more precise than in other studies.…”

“…2 The importance of a re-consent before final release of the UCB sample for allogeneic transplantation single-step two-step concern about misuse doesn't matter n=26 n=154 n=107 Fig. 3 Different attitudes, concerns and consent procedures before release of a rejected UCB sample for research purposes investment would rather lead to fewer UCB donors from the population, considering that a third of them are of immigrant background with most probably limited financial resources [4].…”

“…They provide stem cell transplants with rare HLA alleles and haplotypes that are scarcely found in bone marrow registries [4]. However, far more UCB units have been stored in private institutions.…”

“…9 and hence it is likely that the Arab ethnicity is underrepresented in the western cord blood banks. Many western banks now aim at recruiting nonCaucasian individuals for cord blood units and a recent study showed that some DRB1 alleles occurred at higher frequency in cord blood banks than unrelated donor registries, 14 likely because of increased recruitment of cord blood donation from ethnic minorities. In our study, 96 of the 97 reported patients received units from western cord blood banks, and 47% of the units were one-Ag mismatched.…”

“…Thus, many such countries are now supporting the establishment of national cord blood banks to enhance the probability of finding suitable units for their nationals who need SCT but have no suitable matched family donor. [11][12][13][14] In Saudi Arabia, there is currently no national donor program registry, and until September 2008 there was no national cord blood bank. The first case of UCB transplant in Saudi Arabia was performed in January 2003 at King Faisal Specialist Hospital and Research Center (KFSHRC), and until the opening of the Saudi National Bank, all the units were procured from western banks.…”
