Background Diagnosis of subscapularis (SSC) tendon lesions on magnetic resonance imaging (MRI) can be challenging. A small coracohumeral distance (CHD) has been associated with SSC tears. This study was designed to define a specific threshold value for CHD to predict SSC tears on axial MRI scans. Methods This retrospective study included 172 shoulders of 168 patients who underwent arthroscopic surgery for rotator cuff tear or glenohumeral instability. Diagnostic arthroscopy confirmed an SSC tear in 62 cases (36.0%, test group a), rotator cuff tear tears other than SSC in 71 cases (41.3%, control group b) and glenohumeral instability without any rotator cuff tear in 39 cases (22.7%, zero-sample group c). All patients had a preoperative MRI of the shoulder (1.5T or 3T). Minimum CHD was measured on axial fat-suppressed proton density-, T2-, or T1-weigthed sequences. Receiver operating characteristics analysis was used to determine the threshold value for CHD, and sensitivity and specificity were calculated. Results CHD measurement had a good interobserver reliability (Intraclass correlation coefficient 0.799). Mean CHD was highly significantly ( P < .001) less for test group a (mean 7.3 mm, standard deviation ± 2.2) compared with control group b (mean 11.1 mm, standard deviation ± 2.3) or zero-sample group c (mean 13.6 mm, standard deviation ± 2.9). A threshold value of CHD <9.5 mm had a sensitivity of 83.6% and a specificity of 83.9% to predict SSC tears. Conclusion A CHD <9.5 mm on MRI is predictive of SSC lesions and a valuable tool to diagnose SSC tears.

Çalışmamızdaki amacımız, omuz sıkışmasında etkili olduğunu düşündüğümüz korakoid morfolojisini ve korakohumeral mesafeyi ortaya koymaktır. Gereç ve Yöntem: Her iki omuzda korakoglenoidal aralık tipi, korakoid açılanması ve korakohumeral mesafeler ölçüldü. Olguların yaş, cinsiyet, taraf, boy uzunluğu ve el dominansı ile elde edilen sonuçlar arasındaki korelasyonlar değerlendirildi. Bulgular: Her iki omuz korakohumeral mesafe değerleri erkeklerde kadınlardan daha yüksek bulundu (p<0,05). Yaş ile korakoid açılanma ve korakohumeral mesafe değerleri arasında negatif korelasyon tespit edildi (p<0,05). Olguların boy uzunluklarıyla korakohumeral mesafe değerleri arasında pozitif korelasyon görüldü (p<0,05). Her iki cinsiyette açılanma değerleriyle korakohumeral mesafe değerleri arasında herhangi bir korelasyon tespit edilmedi. Sonuç: Çalışmamız, korakoid açılanması ve korakohumeral mesafe değerlerini ortaya koymanın yanısıra, el dominansı ve korakoid açılanmanın korakohumeral mesafe üzerinde etkisi olmadığını göstermiştir.

BackgroundThis study proposed to investigate whether high-sensitivity C-reactive protein (hs-CRP) is an independent risk factor for long head of biceps tendon (LHBT) tear and whether hs-CRP can increase accuracy in diagnosing LHBT tear.MethodsThis study involved 582 shoulders of 557 consecutive patients who received arthroscopic examinations at the authors’ institution between January 2010 and July 2018. The strengths of associations between LHBT tear and various factors were determined by calculating the odds ratios (ORs), with 95% confidence intervals (CIs), using logistic regression analyses. The studied variables were demographic, physical, social, metabolic, comorbidity, hs-CRP, and pain on a visual analog scale (VAS) factors, as well as those related to rotator cuff tear (RCT). Significant factors in the multivariable logistic analysis were evaluated to determine their diagnostic values, including their likelihood ratios and post-test probabilities for LHBT tear.ResultsIn the multivariable analysis, five variables were significant: age, retraction degree of Patte, subscapularis tendon tear, hs-CRP > 1 mg/L, and pain VAS (p ≤ 0.01). The best combination of determinations for diagnosing LHBT tear, which yielded a strong positive likelihood ratio of 19.07 and a high post-test probability of 96%, was age ≥ 67 years, subscapularis tendon tear, grade of Patte ≥2, hs-CRP > 1, and pain VAS ≥ 7.ConclusionsSerum hs-CRP > 1 mg/L is an independent risk factor for LHBT tear, along with the expected risk factors of age, subscapularis tendon tear, retraction degree of Patte, and pain VAS. Serum hs-CRP > 1 mg/L increases the diagnostic accuracy for LHBT tear.Level of evidenceLevel IV, Clinical case series.
